胱抑素C与原发性高血压患者室壁厚度及心功能不全的相关性

【摘要】 目的探讨胱抑素C水平和原发性高血压患者心肌肥厚及心功能不全的关系。方法纳入122例原发性高血压住院患者,男性38例,女性84例,根据是否合并心衰分为心衰组12例和心功能正常组110例,检测其左心室壁厚度、左心室射血分数、胱抑素C水平和肌酐水平,运用Pearson法分析胱抑素C水平和左心室壁厚度、左心室射血分数及心衰的相关性。结果与心功能正常组相比,心衰组患者胱抑素C水平(P<0.01)和左室壁厚度(P=0.035)明显增加,而肌酐水平差异无统计学意义(P=0.466)。所有患者中胱抑素C水平和室壁厚度(r=0.33,P<0.01)呈正相关,与LVEF呈负相关(r=-0.272,P<0.05)。结论胱抑素C水平升高与原发性高血压患者心肌肥厚和心功能不全的发生有关,其影响独立于肾功能而存在。

【Abstract】 Objective To know the association between left ventricular hypotrophy,heart failure and Cystatin C level in essential hypertension patients. Methods The study included 122 hypertension patients( male 38,female 64) who were divided into two groups by cardiac function as group of cardiac dysfunction(12 cases) and group of normal(110 cases). The clinical data such as left ventricular wall thickness,left ventricular ejection fraction,serum concentration of cystatin C and creatinine were detected,and the correlation of cystatin C levels and left ventricular wall thickness,left ventricular ejection fraction and cardiac failure was analysed by Pearson method. Results①Compared with group of normal,Cystatin C concentration(P < 0. 01)and left ventricular thickness(P = 0. 035)were significantly increased,but creatinine showed no obvious change(P = 0. 466). ②There was a positive correlation between Cystatin C concentration and left ventricular thickness( r = 0. 33,P < 0. 01),and negative correlation between Cystatin C level and LVEF( r =- 0. 272,P < 0. 05). Conclusion Increased serum Cystatin C concentration is involved in cardiac hypertrophy and heart failure in essential hypertension patients,which is independent of renal insufficiency.
