胰岛素强化治疗对初诊断2型糖尿病患者胰岛功能的影响

【摘要】 目的观察短期胰岛素强化治疗对初诊断2型糖尿病患者胰岛α细胞及β细胞功能的影响。方法选取初诊2型糖尿病患者28例,给予餐时胰岛素加基础胰岛素强化治疗(三餐前速效人胰岛素类似物+甘精胰岛素)10 d,治疗前后均行标准馒头餐试验,治疗前后均检测空腹、餐后30 min及餐后120 min的静脉血糖、C-肽及胰高血糖素。结果与强化治疗前相比,治疗后患者各时点静脉血糖明显下降,C-肽曲线下面积增加,胰高血糖素曲线面积下降,各时点C-肽/胰高血糖素比升高(P<0.05)。强化治疗后HOMA-IR指数降低(P<0.05)。结论胰岛素短期强化治疗可改善初诊2型糖尿病患者的β细胞及α细胞功能。

【Abstract】 Objective To investigate the changes of α and β islet cell function of newly diagnosed type 2 diabetics subjected to short- term intensive insulin therapy. Methods Twenty- eight newly diagnosed type 2 diabetics were enrolled and given intensive insulin therapy( glargine insulin plus short- acting insulin). Steamed bread meal test was performed in all patients,and levels of serum glucose,C-peptide and glucagon before and after the intensive insulin therapy were measured and compared. Results Compared with those before the intensive treatment,blood glucose,area of curve of glucagon,and index of HOMA- IR decreased remarkably after the intensive treatment(P <0. 05),while area under curve of C- peptide and C- peptide /glucagon ratio increased significantly(P <0. 05). Conclusion Short- term intensive insulin therapy can improve the function of both α and β islet cells in newly diagnosed type 2 diabetics.
