高分辨在先天性外耳道闭锁诊断中的应用研究
Application Research of High-resolution CT in Diagnosis of Congenital Aural Atresia
【摘要】 目的探讨高分辨CT扫描在先天性外耳道闭锁患者诊断中对于中耳、内耳结构判断的应用价值。方法对41例64耳先天性外耳道闭锁的患者进行高分辨的CT检查,观察中耳的结构面神经、锤砧骨、镫骨、乳突气化、圆床、鼓窦,以及内耳的结构耳蜗、半规管、内听道,并对II级和III级的耳廓畸形患者的上述结果进行比较。结果对41例64耳的高分辨CT结果显示,面神经走行、位置正常者42耳,发现异常者22耳,占34%;锤砧骨形态正常1耳,发现异常者63耳,占99%;镫骨正常显示清楚的31耳,显示异常的33耳,占51%;乳突气化形态正常60耳,发现异常者4耳,占6%;鼓窦形态正常63耳,发现异常者1耳,占2%;圆床形态正常61耳,发现异常者3耳,占5%。41例64耳的内耳高分辨CT结果显示,61耳的耳蜗结构形态正常,3耳发现异常;57耳的半规管结构形态正常,7耳发现异常;60耳的耳蜗结构形态正常,1耳发现异常。对上述中耳、内耳结果正常以及异常的II级和III级的患者进行统计学分析,显示有良好的相关性。结论高先天性外耳道闭锁患者中耳分辨CT可以明确的判断先天性外耳道闭锁患者的中耳、内耳结构,内耳的异常与耳廓畸形的程度有良好的相关性,高分辨CT应为先天性外耳道闭锁患者的常规检查。
【Abstract】 Objective to study the high resolution CT scanning in the diagnosis of patients with congenital external auditory meatus atresia in middle ear, inner ear structure judgment application value.Methods 41 cases of 64 ears congenital external auditory meatus atresia of patients with high resolution CT examination, to observe the structure of the middle ear facial nerve, hammer incus, stapes, mastoid gasification, round bed, drum sinus, and the structure of the inner ear, the cochlea, semicircular canal hear the word, and the level II and III of the auricle malformation patients results were compared with the above.Results 64 ears of 41 cases of high resolution CT results show that for facial travel, position normal 42 ears, found 22 ears middel, accounted for 34%;Hammer incus form normal 1 ears, found 63 ears middel, accounted for 99%; Stape normal display clear and ears, display abnormal and ears, accounted for 51%; Mastoid gasification form normal 60 ears, found that 4 ears middel, accounted for 6%; Drum sinus form normal 63 ears, found one ear was abnormal, accounted for 2%; Round the bed form normal 61 ears, found that was abnormal 3 ears, accounted for 5%.41 cases of 64 ears patients whose high resolution CT results showed that 61 patients with ear cochlea structure form normal, 3 patients with ear abnormal; 57 patients with ear the semicircular canal structure form normal, 7 patients with ear abnormal; 60 patients with ear cochlea structure form normal, 1 ear abnormal patients.On the above middle ear, inner ear results normal and abnormal level II and III of the patients the statistical analysis showed good correlation.Conclusion high congenital external auditory meatus blocking patients middle ear resolution CT can clear judgment congenital external auditory meatus atresia of patients with middle ear, inner structure, the inner ear, the anomaly and auricle deformed degree have good correlation, high resolution CT should be congenital external auditory meatus blocking patients with routine inspection
