【摘要】 目的研究应用脑梗死危险度分级管理结合延伸卒中单元(extended stroke unit,ESU)管理出院后脑梗死患者的疗效,探讨该模式的相关经验。方法 2010年1月—2011年9月在我院神经内科住院的符合研究标准的103例急性脑梗死患者纳入研究,住院期间均给予卒中单元管理治疗,出院时随机分入研究组和对照组,分别给予分级管理结合ESU管理,对照组给予常规管理。发病3个月和6个月时测美国国立卫生研究院卒中量表(NIHSS)、Barthel Index(BI)评分,调查高血压、高血糖和高血脂控制率;评估疗效。结果干预6个月时研究组患者的高血压和糖尿病控制明显好于对照组;半年时,NIHSS明显低于对照组,而其BI指数得分明显高于对照组,差异均有统计学意义(P<0.05)。结论分级管理结合ESU管理模式较常规模式能集中有限医疗资源,更好促进控制脑梗死危险因素,降低再发风险,促进患者功能恢复,值得进一步深入研究。更多还原

【Abstract】 Objective To investigate the effect of extended stroke unit service and classification management on stroke patients.Methods One hundred and three inpatients with stroke were randomly divided into the extended stroke unit group and the control group.The primary outcome was assessed by National Institutes of Health stroke scale(NIHSS) and Barthel Index(BI) after 3 months and 6 months.The controling rate of hypertension,diabetes mellitus and hyperlipemia were observed.Results There were significant differences in the controlling rate of hypertension,diabetes mellitus between two groups.The scores of Barthel Index in extended stroke unit group was higher than that in control group,while scores of NIHSS in extended stroke unit group was lower than that in control group(P<0.05).Conclusion Classification management and extended Stroke Unit could decrease risk factor of cerebral infarction and improve functional outcome.更多还原
