原发性肾小球疾病患者肾组织Kim-1 mRNA表达的临床意义
Clinical significance of Kim-1 mRNA expression in renal tissue of patients with primary glomerulopathy
【摘要】 目的探讨原发性肾小球疾病(PG)患者肾组织肾损伤因子-1(Kim-1)mRNA的表达及其临床意义。方法收集22例PG患者病例资料,常规检测血肌酐、血尿素、血浆白蛋白,按简化MDRD方程计算估测肾小球滤过率(eGFR);肾穿刺活检进行肾脏病理评分,定量PCR测定肾组织Kim-1 mRNA水平。结果按eGFR水平将PG患者分为两组,结果发现,PGⅠ组eGFR<90 mL/min/1.73 m2,血肌酐对数值、血尿素和肾脏病理评分对数值均明显高于PGⅡ组;且PGⅠ组Kim-1 mRNA对数值显著低于PGⅡ组。Kim-1 mRNA表达水平的对数值与血肌酐对数值或肾脏病理评分对数值之间、肾脏病理评分对数值与eGFR之间均呈负相关,而Kim-1 mRNA表达水平对数值与eGFR之间则呈正相关(P<0.05)。结论 Kim-1 mRNA在eGFR<90 mL/min/1.73 m2者表达较低,肾组织Kim-1 mR-NA表达水平对数值对于评价PG进程中肾损害及其修复程度可能具有一定价值。
【Abstract】 Objective To explore the expression of kidney injury molecule-1(Kim-1) mRNA in renal tissue and its clinical significance in primary glomerulopathy(PG) patients.Methods Serum creatinine(Scr),blood urean(Urea) and plasma Albumine in 22 PG patients were detected by automatic biochemistry analyzer.Estimation of glomerular filtration rate(eGFR) was calculated via simple MDRD formula.Renal pathological scoring(RPS) was assessed by renal needle biopsy.The Kim-1 mRNA expression level of renal tissue was measured by real-time quantitative PCR.Results All cases were divided into two groups according to the eGFR level.The logarithm of Scr,Urea and RPS in PG I group(eGFR < 90 mL/min/1.73 m2) was obviously higher than that in PG II group.However,the logarithm of Kim-1 mRNA expression in PG I group was lower than PG II group.The relationship between logarithm of Kim-1 mRNA expression and log Scr or log RPS and between log RPS and eGFR showed negative correlations,whereas the relationship between the logarithm of Kim-1 mRNA expression and eGFR showed positive correlations(P<0.05).Conclusion The low Kim-1 mRNA expression is detected in renal tissue of PG patients when eGFR is less than 90 mL/min/1.73 m2.The logarithm of Kim-1 mRNA expression might be useful for the evaluation of renal injury and recovery in PG progression.
