2型糖尿病下肢血管病变与血清尿酸的关系

【摘要】 目的:研究2型糖尿病(T2DM)合并下肢血管病变患者的临床特点及其与血清尿酸(SUA)的关系。方法:将94例住院T2DM患者根据有无合并下肢血管病变分为合并下肢血管病变组和无下肢血管病变组,记录患者身高、体重、腰围、臀围,进行空腹血糖(FBG)、血总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、甘油三酯(TG)、血清尿酸(SUA)、空腹胰岛素(FINS)测定,计算体重指数(BMI)、腰臀比(WHR)、胰岛素抵抗指数(HOMA-IR)。对两组的一般临床资料进行统计学分析。结果:2型糖尿病合并下肢血管病变组BMI、WHR、FINS、HOMA-IR、LDL-C、TG、SUA较无下肢血管病变组升高(P<0.05),而FBG、TC、HDL-C两组差别无统计学意义(P>0.05)。经年龄、性别、血脂调整后血尿酸水平高者发生糖尿病下肢血管病变的危险性提高(OR=1.113,P<0.05)。结论:2型糖尿病合并下肢血管病变组血脂异常、胰岛素抵抗更明显,SUA的水平显著升高。

【Abstract】 Objective:To investigate the relationship between between lower extremities arterial disease( LEAD) in type 2 diabetes mellitus and serum level of uric acid(SUA) .Method:94 cases of T2DM were divided into 2 groups with LEAD and without LEAD. Their body height,weight,waist,hip were recorded ;the levels of fasting blood glucose (FBG),serum total cholesterol(TC),serum high density lipoprotein (HDL-C),serum low density lipoprotein (LDL-C),serum triglyceride(TG),SUA and fasting serum insulin(FINS) were determined,body mass index(BMI),the waist to hip ratio( WHR)and insulin resistance index (HOMA-IR)were calculated. Result:The levels of BMI,WHR,FINS,HOMA-IR,LDL-C,TG and SUA increased significantly in LEAD group compared with non- LEAD group (P<0.05),the levels of FBG,TC and HDL-C had no statistical significance between two groups (P>0.05).SUA was a risk factor of LEAD after age,gender,lipid-adjusted(OR=1.113,P<0.05).Conclusion:LEAD in T2DM patients have remarkable dyslipidemia,insulin resistance and higher SUA.
