【摘要】 目的探讨中晚期特殊部位异位妊娠的主要临床特征,为临床诊治提供参考。方法回顾性分析中南大学湘雅医院2002年1月-2010年6月收治的中晚期特殊部位异位妊娠患者的临床资料。结果异位妊娠发展至中晚期的比率为7.41%,前三位依次为宫角妊娠合并宫内妊娠(5/16例)、宫角妊娠(4/16例)和子宫瘢痕部位妊娠(4/16例)。75%(12/16例)患者出现不规则的腹痛及内出血表现,87.5%(14/16例)的患者有子宫不规则增大和胎儿在宫腔内占据位置的异常。延迟至产后诊断的患者均以胎盘因素性产后出血为主要临床表现。且术前和术后诊断的患者平均总失血量分别高达1 485 mL和3 652 mL,明显高于孕期诊断的患者。结论不规则的腹痛、严重产后出血、子宫不规则增大和胎儿在宫腔内占据位置的异常是中晚期特殊部位的异位妊娠患者的主要临床特征。更多还原

【Abstract】 【Objective】To analysis the clinical feature on the special-site ectopic pregnancy during second and third trimester and provide the clinical references.【Methods】16 cases of special-site ectopic pregnancies during second and third trimester were analyzed retrospectively from January 2002 to June 2010 in xiangya hospital of central southern university.【Results】 The ratio of special-site ectopic pregnancy prolonged to second and third trimester was about to 7.41%(16/216 cases).There were ranked to cornual pregnancy with intrauterine pregnancy(5/ 16 cases),cornual pregnancy(4/16 cases) and uterine scar pregnancy(4/16 cases),respectively.The 75%(12/16 cases) of patients with irregular abdominal pain and bleeding in abdominal cavity,87.5%(14/16 cases) cases had the sign of symmetrical enlarge of uterine and fetus in uterine cavity anomaly.And,the patients diagnosed in postpartum had been symptom with severe postpartum hemorrhage.There were remarkable differences calculated the blood loss of the patients diagnosed in pregnant stage with 220 mL,prior to operation with 1485 mL and after postpartum with 3652 mL.【Conclusion】The special-site ectopic pregnancy proceeding to second and third trimester may have the main clinical manifestations including to irregular abdominal pain,severe postpartum hemorrhage,symmetrical enlarge of uterine and fetus in uterine cavity anomaly.更多还原
