四种方法检测婴幼儿腹泻A组轮状病毒的评价

【摘要】 目的:对A组轮状病毒(RV)所致婴幼儿腹泻的四种不同的检测方法进行评价。方法:同时采用胶体金免疫层析法、酶联免疫吸附试验(ELISA)、聚丙烯酰胺凝胶电泳法(PAGE)、逆转录PCR(RT-PCR)对200份腹泻患儿的标本进行RV检测并进行比较。结果:四种方法的阳性检出率分别为:49%,50%,37%和42.5%。以PAGE为检测的最终标准,RT-PCR的敏感性为100%,特异性和准确性均在90%以上;胶体金和ELISA法的敏感性、准确性分别在95%和85%以上,特异性均为78.57%。结论:胶体金和ELISA法检测RV能够满足临床上对RV快速筛检的要求;RT-PCR法的敏感性和特异性均最高,在无菌体液和环境样本的检测上具有不可比拟的优势;PAGE法特异性高但操作繁琐,较适合RV的流行病学分析。

【Abstract】 Objective:To evaluate four methods in testing Rotavirus group A in infant diarrhea.Methods: Rotavirus group A in 200 specimens of infant diarrhea was tested by the colloidal gold immunochromatographic assay(ICA),enzyme-linked immunosorbent assay(ELISA),polyacrylamide gel electrophoresis method(PAGE),reverse transcription-polymerase chain reaction(RT-PCR) at the same time,then the test results were compared.Results: The positive rates of four methods were 49%,50%,37% and 42.5% respectively.With the PAGE results as the final standard,RT-PCR sensitivity was 100%,specificity and accuracy were above 90%;the sensitivity and accuracy of ICA and ELISA were above 95% and 85%,respectively,the specificity were all 78.57%.Conclusion: The method of ICA and ELISA can satisfy the clinical rapid screening of Rotavirus;PT-PCR method have the highest sensitivity and specificity,it has incomparable advantages,especially in testing the sterile fluid and environment samples;PAGE as a standard method,usually used for epidemiological analysis of Rotavirus.
