奇异变形杆菌耐药性和超广谱β-内酰胺酶基因型研究
Study on genotypes of extended spectrum β-lactamases and antibiotic resistance of Proteus mirabilis strains
【摘要】 目的:了解奇异变形杆菌临床分离株对抗菌药物的体外敏感性和临床分布,研究奇异变形杆菌超广谱β-内酰胺酶基因类型分布。方法:用K-B法对105株奇异变形杆菌进行药物敏感性检测;PCR法检测超广谱β-内酰胺酶基因,并对PCR结果进行测序分析。结果:奇异变形杆菌菌株对亚胺培南、美洛培南、头孢西丁100%敏感;而对环丙沙星、左旋氧氟沙星和第三代头孢菌素的耐药率保持在较低水平;基因型分析显示有17株携带CTX-M基因,其中3株奇异变形杆菌基因型为CTX-M-14型,14株为CTX-M-15型。结论:奇异变形杆菌ESBLs检出率仍较低;17株产超广谱β-内酰胺酶奇异变形杆菌对多种抗菌药物耐药,其基因型为CTX-M-15或者CTX-M-14;需加强耐药监测。
【Abstract】 Objective:To understand the antibiotic resistance,distribution of nosocomial infections and genotypes of extended spectrum β-1actamases of Proteus mirabilis strains isolated from clinical specimens.Methods: A total of 105 strains of Proteus mirabilis were analysed for their antimicrobial susceptibility.The gene of ESBLs-producing isolates were detected by PCR and then sequenced.Results: None of these Proteus mirabilis isolates was found resistant to imipenem,mempenem and cefoxitin,while their drug resistance rates to ciprofloxacin,levofloxacin and the third-generation cephalosporins were at low level,17(16.2%) strains were identified as ESBls producers.The genotypes of these strains included CTX-M-14(14/17),CTX-M-15(3/17).Conclusion: The detection rate of ESBL-producing Proteus mirabilis was still low.17 strains of ESBL-producing Proteus mirabilis,with the genotype of CTX-M-15 and CTX-M-14,were resistant to many antibacterial agents.More attention should be paid to survey such strains.
