VEGF和端粒酶检测对恶性腹水的诊断价值

【摘要】 目的探讨VEGF和端粒酶检测对恶性腹水的诊断价值。方法选择确诊为恶性腹水的患者60例和同期入院确诊为良性腹水的患者75例,采用TCT方法制片,SABC法检测细胞VEGF和端粒酶的表达,并统计其染色得分。利用SPSS 11.5绘制ROC曲线,计算曲线下面积,根据ROC曲线判断最佳诊断价值(灵敏度与特异度之和最大)时候的界值,计算阳性预测值、阴性预测值、阳性似然比、阴性似然比和尤登指数(Youden index,YI)。结果 VEGF和端粒酶曲线下面积分别为为0.953和0.957,VEGF最佳诊断界限得分为300分,端粒酶最佳诊断界限得分为320分,在此界限下的诊断灵敏度分别0.900和0.933,特异度为0.920和0.933。结论对腹水细胞进行细胞化学染色检测VEGF和端粒酶染色得分能够很好地鉴别诊断腹水的良性、恶性,为临床准确诊断腹水的性质提供依据。

【Abstract】 Objective To investigate the diagnostic value of VEGF and telomerase activity in malignant ascites.Methods 60 patientsdiagnosed with malignant ascites and 75 cases diagnosed as benign ascites were as samples,the ascites cells were harvested.TCT were used to make cells smears,the expression of VEGF and telomerase was detected by SABC,the Total staining score is calculated.Using Spss 11.5 to draw the ROC curve.Determine the boundary value of best diagnostic performance,and calculate the positive predictive value,negative predictive value,positive likelihood ratio,negative likelihood ratio and Youden index(YI).Results The area under the ROC curve of VEGF and telomerase were 0.953,0.957.the best diagnostic boundary of VEGF and telomerase is 300 points and 320 points.In this limit the diagnostic sensitivity of them were 0.900,0.933,and specificity were 0.920,0.933.Conclusion Detected the espression of VEGF and telomerese of ascites by cytochimistry staining allows a good diagnosis of benign and malignant ascites.It Provide the basis for the accurate clinical diagnosis of ascites.
