耐甲氧西林金黄色葡萄球菌SCCmec基因分型与耐药性研究

【摘要】 目的了解临床MRSA菌株SCCmec基因型分布和各种基因型的耐药特征。方法采用应用多重聚合酶链式反应(PCR)技术对MRSA进行SCCmec基因分型;采用K-B纸片扩散法检测MRSA抗菌药物敏感性。结果 65株MRSA菌株中包括SCCmecⅢ型60株(92.31%),SCCmecⅣa型4株(6.15%),未分型1株(1.54%)。耐药性分析显示所有菌株对万古霉素、利奈唑烷、替考拉宁和奎奴普丁/达福普汀完全敏感,对氯霉素耐药率较低,各型别间无显著差异(P>0.05)。对其它非β-内酰胺类的抗菌药物,SCCmecⅢ型MRSA菌株表现为多重耐药,其耐药率显著高于SCCmecⅣa型,其中对加替沙星、庆大霉素,利福平,四环素的耐药率有显著差异(P<0.05)。结论 SCCmecIII型菌株为临床分离MRSA主要流行菌株,该型菌株对多种抗生素呈多重耐药。多重PCR对MRSA进行SCCmec基因分型适合临床实验室。

【Abstract】 Objective To investigate the distribution and antimicrobial resistance of different methicillin-resistant staphylococcus aureus(MRSA) SCCmec genotype in Binzhou Medical University Hospital.Methods SCCmec types were determined by Multiplex PCR methods.The susceptibility to antimicrobial agents was tested by Kirby-Bauer Results Among the 65 isolates of MRSA,60(93.75%) isolates were belonged to SCCmecIII,4(6.25%) isolates were belonged to SCCmecIVa,only one isolate could not be typed.Drug resistance analysis showed the resistance rates to chloramphenicol of all strain was lower,to vancomycin,linezolid,teicoplanin and quinupristin were completely sensitive,there were not significant difference among the SCCmec genotype(P<0.05).The resistance rates of SCCmecⅢ to other non-β-lactamase antibiotics of the strains was higher than SCCmecⅣa.The strains of SCCmecⅢ were multi-resistant and resistance rates of clarithromycin,Gatifloxacin,rifampin and tetracycline were manifest higher than SCCmecIVa(P<0.05), Conclusion SCCmecIII genotype was the major epidemic isolates in Binzhou Medical University Hospital.and showed multiple resistance to antibiotics.Multiplex PCR is capable to identify the genotypes of SCCmec in MRSA.
