双侧多焦点与单眼视设计单焦点人工晶状体眼的立体视功能
Comparison of Stereopsis between Bilateral Multifocal Lens and Monovision Pseudophakia Eyes
【摘要】 【目的】探讨双侧多焦点人工晶状体眼与单眼视设计双侧单焦点人工晶状体眼的立体视功能。【方法】行双眼超声乳化白内障摘除联合人工晶体植入手术的患者,分为3组:多焦组、单焦组和单眼视组。多焦组双眼植入Array多焦点人工晶状体,共27例;单焦组双眼植入单焦点人工晶状体,共26例;单眼视组,一眼按术后裸眼正视,另一眼按术后裸眼为-1.75 m-1近视植入相应度数单焦点人工晶状体,共31例。记录远、近视力及远、近立体视锐度。【结果】术后多焦组和单焦组最佳矫正远视力(LogMAR,以下同)分别为-0.03±0.08和-0.03±0.07,单眼视组拟正视眼裸眼远视力是-0.04±0.08,三组比较P>0.05;术后单焦组和多焦组近视力及单眼视组拟视近眼裸眼近视力分别为0.52±0.14、0.31±0.13和0.24±0.10,三组比较P<0.001。多焦组和单焦组最佳矫正近视力分别为0.13±0.09和0.05±0.09,单眼视组拟视近眼裸眼近视力为0.24±0.10,三组比较P<0.001。多焦组、单焦组和单眼视组远立体视锐度分别为60″、60″和400″,近立体视锐度分别为50″、100″和50″。【结论】双侧多焦点人工晶状体眼近视力和近立体视功能优于双侧单焦点人工晶状体。单眼视设计的双侧单焦点人工晶状体眼能同时获得良好的裸眼远、近视力,裸眼下近立体视功能正常,但远立体视功能下降。
【Abstract】 【Objective】 To evaluate vision acuity and stereopsis after bilateral implantation of array refractive multifocal intraocular lens(IOL) and of monovision pseudophakia.【Methods】 Stereoacuity,distance and near visual acuity were examined in 27 patient with bilateral implantation of multifocal IOL and 26 patients with monofocal IOL and in 31 patients with monovision pseudophakia.In monovision patients,implantation of IOL calculated to achieve emmetropia was performed in one eye,then a IOL calculated to yield a spherical equivalent of-1.75 m-1 was implanted into the second eye.【Results】 There was no significant difference in best corrected distance vision acuity in 3 groups(P > 0.05).Distance-corrected near vision acuity(DCNVA) in multifocal group was better than monofocal group(P < 0.001).In monovision patients,uncorrected distance vision acuity(UCDVA) and uncorrected near vision acuity(UCNVA) of the fellow eyes were-0.04 ± 0.08 versus 0.34 ± 0.13 and 0.56 ± 0.12 versus 0.24 ± 0.10(P < 0.001).Near stereopsis with DCNVA was significant better in the multifocal IOL than monofocal IOL(P < 0.05).In monovision patients,the distance stereoacuity was 400″ and near stereoacuity without correction was 50″ which was similar to that of multifocal group with DCNVA(P > 0.05).【Conclusion】 DCNVA and stereoacuity with DCNVA in multifocal group were better than monofocal group.In monovision group,patients can get good UCDVA and UCNVA,distance stereoacuity was injuried and near stereoacuity was improved.
