太仓市1727例体重指数正常女性跟骨超声骨密度测量分析
Analysis on 1727 cases of quantitative ultrasound measurement of feminine calcaneus with normal BMI in Taicang
【摘要】 目的旨在探讨太仓地区体重指数(BMI)正常女性跟骨超声骨质量特点。方法使用定量超声测定1727例BMI正常女性右侧跟骨超声骨质量指标,回顾性分析各年龄组超声指标的特点及其与体质量的相互关系。结果 (1)53岁后的各年龄组超声指标呈现明显逐年下降趋势,较38岁前各年龄组差异有统计学意义(P<0.01);(2)分别于18~22岁和38~42岁出现两个骨峰值;(3)年龄与声速(SOS)、传播指数(TI)、骨超声评价指数(OSI)及BMI有相关性(r=-0.279、-0.163、-0.212、0.238,P均<0.01);(4)以18~23岁为骨峰值组,53~57岁、58~62岁和≥63岁三组的OSI-TM(%,s)分别为(93.50,-0.80)、(89.03,-1.34)、(84.84,-1.86);(5)SOS自38岁后逐年下降,不受身高、体重、BMI影响;(6)年龄<43岁,OSI=2.676+0.082×年龄(t=5.912,P<0.01),而年龄≥43岁,OSI=2.758-0.082×年龄(t=-5.912,P<0.01)。结论太仓地区女性超声骨质量随着年龄变化,有两峰表现;应加强43岁前、处于孕产期和绝经期女性的骨健康教育;适当地增加体质量可提高骨密度,但不能阻止骨量减少的发生;SOS和OSI互参,可提供更多骨质量的信息。
【Abstract】 Objective To analyze the character of bone mineral density by quantitative ultrasound(QUS)of feminine calcaneus with normal body mass index(BMI)in Taicang.Methods 1727 women′s right heels with normal BMI were measured by QUS.Character of the QUS indexes in the different age groups and the relationship between the indexes and BMI were retrospectively analyzed.Results (1)All QUS indexes of groups after 53 were decreasing annually and after compared with groups before 38 the difference was significant(P<0.01);(2)peak bone mass(PBM)occurred in the groups of 18-22 years old and 38-42 years old;(3)There was a good relationship between age and QUS indexed[γ=-0.279(with SOS),-0.163(with TI),-0.212(with OSI),P<0.01];(4)OSI-TM(%,s)in groups of 53-57 years old,58-62 years old and≥63 years old respectively was(93.50,-0.80),(89.03,-1.34),(84.84,-1.86)by using PBM of 18-23 years old to compute.(5)SOS were declining annually without influence of height,weight and BMI.(6)OSIage<43=2.676+0.082×age(t=5.912,P<0.01),and OSIage≥43=2.758-0.082×age(t=-5.912,P<0.01).Conclusions (1)The QUS indexes are declining annually with aging.There is a characteristic two peak bone mass.Health service about bones should be enhanced in the periods of pregnancy,maternity and menopause.(2)Weight and BMI both are useful to increase BMD but cannot stop its declining.(3)SOS companied with OSI can give more information of bone quality.
