50岁以下BPH患者前列腺体积与代谢综合征的关系
Relationship between metabolic syndrome and prostate volume in BPH under 50 years old
【摘要】 目的代谢综合征(MS)被认为是老年良性前列腺增生症(BPH)患者的重要影响因素,但针对相对年轻的国人中前列腺体积与代谢综合征的关系,尚无相关报道。本研究旨在分析MS与50岁以下国人前列腺体积的关系。方法对55例年龄小于50岁(含50岁)的BPH患者临床资料进行回顾性研究,经直肠超声检查(TRUS)确定前列腺体积,采集包括身高、体重、空腹血糖、餐后2h血糖、糖化血红蛋白、TC、TG、HDL-C、LDL-C等数据进行分析,评价MS及MS相关指标与前列腺体积的关系。结果伴发MS的前列腺增生症患者与无伴发MS组之间前列腺体积无统计学差异(32.5±3.6mlvs30.8±4.5ml,P=0.122)。对MS相关指标的分层研究,体重指数(BMI)异常组(≥25kg/m2)较正常组(<25kg/m2)的前列腺体积明显增大,两者之间具有统计学差异(32.6±4.0mlvs29.2±3.6ml,P=0.007),对组内MS各项指标进行多因素logistic回归法分析,其中仅有BMI成为导致BPH的独立危险因素(P<0.05,OR=2.114,95%CI1.24~3.604);其他如空腹血糖(FPG)、血压(SBP)、空腹血浆TG以及空腹HDL-C等与前列腺体积无明显的关联(P>0.05)。结论相对年轻的BPH患者(<50岁),是否伴发MS对前列腺体积无明显的影响,但体重指数(BMI)却是导致BPH的独立危险因素。体重指数(BMI)应被认为是相对年轻的BPH患者的重要预测指标。
【Abstract】 Objective The metabolic syndrome(MS) is an important cause of benign prostatic hyperplasia(BPH) in old age.However,no studies were focused on the interrelation between MS and prostate volume in relatively young adults.This study was purposed to evaluate the relationship between parameters of MS and prostate volume in men under 50-year-old.Methods Fifty-five cases with BPH were enrolled between August 2007 and November 2011.Clinical data including prostate volume confirmed by transrectal ultrasound,parameters of MS such as TC、TG、HDL-C、LDL-C、BMI,were analyzed retrospectively.The correlation between parameters of MS and prostate volume was evaluated.Results There was no significant difference of prostate volume between BPH with MS and without MS(32.5±3.6 ml vs 30.8±4.5 ml,P=0.122).The prostate volume in subjects with abnormal BMI(≥25 kg/m2) was significantly larger than those with normal parameters(<25 kg/m2)(32.6±4.0 ml vs 29.2±3.6 ml,P=0.007).The logistic regression analysis showed only the BMI had a significantly positive correlation with the prostate volume(P<0.05,OR=2.114,95%CI 1.24~3.604).There were no significant relationship between FPG,SBP,TG,HDL-C and prostate volume(P>0.05).Conclusions In young BPH patients(under 50-year-old),the prostate volume is not related with MS.Abnormal BMI rather than MS,is an important predictive factor for prostate enlargement in young BPH adults.
