自体骨髓干细胞移植治疗新鲜颈中型股骨颈骨折临床研究

【摘要】 目的:探讨自体骨髓干细胞移植治疗新鲜颈中型股骨颈骨折临床疗效。方法:2006年7月-2010年12月对44例新鲜颈中型股骨颈骨折患者施行自体骨髓干细胞移植配合采用牢固内固定AO技术,对照组43例单纯采用AO 3枚空心加压螺钉内固定,两组进行临床研究观察。治疗组与对照组治疗后第12、24个月进行髋关节Harris评分及X线片检查评价疗效。结果:治疗组44例患者治疗后第3月股骨颈骨折愈合38例,未愈合6例;对照组43例患者治疗后第3月股骨颈骨折愈合28例,未愈合15例,两组比较差异有统计学意义(P<0.05);12、24个月后进行髋关节Harris评分治疗组分别为67.21±10.82,86.43±11.52;对照组分别为54.72±12.23,74.32±14.43;两组比较差异有统计学意义(P<0.05)。结论:新鲜颈中型股骨颈骨折患者施行自体骨髓干细胞移植配合采用牢固内固定AO技术方法简便,安全有效,对促进骨折愈合、改善髋关节功能患者依从性好,是治疗新鲜颈中型股骨颈骨折的一种新径。

【Abstract】 Objective:To explore the effects of autologous bone marrow implantation in treating fresh femoral neck fracture.Methods:From July 2006 to December 2010,44 cases of fresh femoral neck fractures received autologous bone marrow implantation combined with AO internal fixation.Another 43 cases were given AO internal fixation only as controls.At the end of 12 and 24 months,the curative effects were evaluated by Harris scoring system and X-ray examination.Results:Three months after the treatment,the fracture was healed in 38 cases and failed in 6 cases,while in controls,the fracture was healed in 28 cases and failed in 15 cases(P<0.05).Harris scores at the end of 12 and 24 months were(67.21±10.82) and(86.43±11.52) in treatment group.In control group,the scores were(54.72±12.23) and(74.32±14.43),respectively.There was significant difference between the two groups(P<0.05).Conclusion:Autologous bone marrow implantation combined with AO internal fixation is simple and effective for fresh femoral neck fracture,which appears to be a new way of treatment.
