冷沉淀在不同温度不同时间放置对Ⅷ因子的影响
Influence of cryoprecipitate placed with different temperature and standing time on Ⅷ factor
【摘要】 目的探讨制备冷沉淀的不同时间及不同温度对Ⅷ因子的影响,为制备冷沉淀选择最佳时间、温度和指导临床输注发挥作用。方法随机抽取6h内制备成的新鲜冰冻血浆(FFP)200mL/袋的A与B,用离心法制备成冷沉淀20~25mL,各自分别抽取2mL放入5个不同编码的无菌试管内。A组(A1、A2、A3、A4、A5)于24℃室温放置,B组(B1、B2、B3、B4、B5)于4℃放置,AB两组分别在0、3、5、7、9h观察测定。结果冷沉淀主要含Ⅷ因子、纤维蛋白原、VWF、第Ⅴ因子以及纤维结合蛋白等,但主要成分是Ⅷ因子,其稳定性差,在24℃的室温和4℃放置,随时间变化而变化。结论控制制备时间、温度和临床使用冷沉淀的放置时间、融解时间及温度、运输过程冷链、确保血液成分有效,对减少血液浪费十分重要。
【Abstract】 Objective To explore the influence of cryoprecipitate prepared with different temperature and standing time on Ⅷ factor for preparation of cryoprecipitate with optimum temperature and standing time,in order to guide clinical transfusions.Methods Two fresh plasmas of 200 mL/bag refrigerated for 6 h were sampled randomly and prepared into 20-25 mL cryoprecipitate using centrifuge method.They were marked with A and B.Five repeats,2 mL for each were taken from A and B,respectively and transferred to sterile tubes.These tubes were named as A1,A2,A3,A4 and A5 for A(group A),and B1,B2,B3,B4 and B5 for B(Group B).Group A and B were placed at 24℃ and 4℃ respectively for standing 0,3,5,7 and 9 h.Results Cryoprecipitate mainly contains Ⅷ factor,fibrinogen,VWF,factor Ⅴ and fibronectin.However,Ⅷ factor was observed with poor stability because it related to 24℃ and 4℃ is varied with standing time.Conclusion For guarantee of effective blood constituents and reducing waste of blood,it is important to control time and temperature for preparation,using clinically cryoprecipitate with properly standing time,dissolving time and temperature,and having cold chain transportation.
