【摘要】 目的分析大分割放疗联合PD方案同步化疗治疗局部晚期非小细胞肺癌的临床疗效及毒副作用。方法资料完整的92例局部晚期非小细胞肺癌患者随机分为对照组和大分割组,大分割组接受X射线照射(DT:62.5Gy/25F/5W)并同步给予PD方案化疗4周期,对照组接受常规X射线照射(DT:66～70Gy/33～35F/7W)并同步给予PD方案化疗4周期,治疗结束后评价疗效和不良反应。结果近期有效率大分割组为86.96%(40/46),高于对照组的78.26%(36/46),P>0.05,差异无统计学意义;常见毒副作用主要为骨髓抑制、放射性肺炎、消化道反应及放射性食管炎,大多为Ⅰ～Ⅱ级,未见Ⅳ级不良反应,两组间比较P>0.05,差异无统计学意义。结论大分割放疗联合PD方案同步化疗是治疗局部晚期非小细胞肺癌的有效手段,能缩短放疗时间,且不加重毒副作用。更多还原

【Abstract】 Objective To observe the efficiency and toxicity of treatment of locally advanced non-small cell lung cancer by hypofractionated conformal radiotherapy concurrent with PD regimen chemotherapy.Methods Ninty two patients with locally advanced non-small cell lung cancer were randomly divided into two groups:46 cases in hypofractionated conformal radiotherapy group(DT:62.5Gy/25F/5W)and 46 cases in control group(DT:66-70Gy/33-35F/7W),all receive 4 circles of treatment of PD regimen chemotherapy in both group concurrently.Efficiency and toxicity were evaluated after the treatment.Results The short-term effective rate in hypofractionated conformal radiotherapy group was 8.70% more than control group with total response rate 86.96%(40/46).No significant difference was shown in statistics(P > 0.05).The major adverse effects were hematological toxicity,radiation pneumonitis,vomiting and radiation esophagitis in grade Ⅰ-Ⅱ,which took a favorable turn after heteropathy.The adverse effects in grade Ⅳ were not turn up.There was no significant difference between two groups(P > 0.05).Conclusion Hypofractionated conformal radiotherapy concurrent with PD regimen chemotherapy shows good efficacy and tolerable toxicity in treatment of patients with non-small cell lung cancer.更多还原
