囊袋张力环在外伤性晶状体半脱位手术中的应用

【摘要】 目的探讨囊袋张力环在挫伤性晶状体半脱位超声乳化摘除晶状体术中的应用价值。方法眼钝挫伤晶状体半脱位患者20例(20只眼),行环形撕囊+超声乳化白内障吸除+囊袋张力环植入+后房型人工晶体植入术治疗。对其临床资料进行回顾性分析。结果术前超声生物显微镜(ultrasound biomicroscopy,UBM)探查外伤眼晶状体悬韧带离断情况,离断口范围为45°至135°。术前矫正视力为光感-0.5,眼压10-29mmHg(1 mmHg=0.133 kPa),平均眼压18.6 mmHg。术后1周矫正视力、眼压、前房深度均较术前明显好转。人工晶状体均正位,无倾斜及明显偏位,未发现张力环引起的并发症。结论在晶状体半脱位超声乳化手术中,囊袋张力环是一种安全有效的白内障手术辅助工具,能够防止术后人工晶状体偏位并抑制囊袋收缩。

【Abstract】 OBJECTIVE To evaluate the application of capsular tension ring(CTR) implantation during phacoemulsification for traumatic subluxated lens.METHODS Twenty cases(20 eyes) of traumatic lens subluxation with implanting CTR combined continues curvilinear capsulorhexis(CCC),phacoemulsification and posterior chamber intraocular lens(IOL) implantation were reviewed.RESULTS Twenty cases(20 eyes) of lens subluxation were subjected to ultrasound biomicroscopy(UBM) for determining the lens ligament detachment,which was 45°-135°.Preoperative corrected visual acuity(VA) ranged from light perception(LP) to 0.5.The preoperative intraocular pressure(IOP) was 10-29mmHg,mean IOP was 18.6 mmHg.CTR was implanted after continues curvilinear capsulorhexis(CCC),phacoemulsification and posterior chamber intraocular lens(IOL) implantation for all patients.Postoperative corrected visual acuity,intraocular pressure,and anterior chamber depth were significantly improved.The IOL in all cases were in the normal position without tilting or decentration.There were no special complications of CTR.CONCLUSIONS The CTR implantation during phacoemulsification is a safe and effective method for traumatic subluxated lens.It can prevent the IOL decentration and decrease the complications.
