声触诊组织量化技术在甲状腺占位性病变的应用

【摘要】 目的探讨声触诊组织量化技术在甲状腺占位性病变的应用价值。方法应用声触诊组织量化技术检测甲状腺占位性病变89个,对病灶不同部位实性部分的剪切波速度进行多次测量,术后进行统计学分析。结果 89个病灶中,良性64个,恶性25个。良性病灶剪切波速度的最大值(Vmax)、最小值(Vmin)、标准差(S)、平均值(Vm)、变异系数(CV)、病灶与正常组织平均值之比(Vma/Vmb)分别为(2.24±0.60)m/s、(1.47±0.46)m/s、(0.26±0.15)m/s、(1.89±0.44)m/s、(14.09±8.73)%、1.02±0.27;恶性病灶分别为(4.80±1.31)m/s、(1.94±0.47)m/s、(0.99±0.33)m/s、(3.05±0.70)m/s、(30.97±8.84)%、1.81±0.57。上述变量良性组与恶性组之间统计学上差异有显著性(t检验,P<0.05)。应用ROC曲线,鉴别良恶性的准确性依次如下:S>Vmax>Vm>Vma/Vmb>CV>Vmin。结论声触诊组织量化技术在甲状腺占位性病变良恶性鉴别方面有较高应用价值。

【Abstract】 Objective To investigate the evaluation of virtual touch tissue quantification in the diagnosis of thyroid space-occupying lesions.Methods Virtual touch tissue quantification were applied in 89 thyroid space-occupying lesions before surgery and the shear wave velocity of the lesions different solid parts were examined repeatly.A postoperative statistical analysis were performed.Results There are 64 benign and 25 maligant lesions in all lesions.The maximum,minimum,standard deviation,mean,coefficient of variation and the ratio(mean of lesions and normal tissue) of benign lesions shear wave velocity repectively is(2.24±0.60)m/s,(1.47±0.46)m/s,(0.26±0.15)m/s, (1.89±0.44)m/s,(14.09±8.73)%,1.02±0.27.The maligant group repectively is(4.80±1.31)m/s,(1.94±0.47)m/s,(0.99±0.33)m/s,(3.05±0.70)m/s,(30.97±8.84)%,1.81±0.57.The benign group and malignant group have statistical difference in above indexes(t test,P<0.05).A ROC curve were used and the accuracy sequence of above indexes in identifying benign and malignant lesions is standard deviation,maximum,mean,ratio(mean of lesions and normal tissue)coefficient of variation,minimum.Conclusions Virtual touch tissue quantification has high value in the differential diagnosis of benign and malignant thyroid space-occupying lesions.
