超声弹性成像、钼靶X线摄影评估绝经后妇女小剂量激素补充治疗对乳腺的影响

【摘要】 目的观察绝经后妇女小剂量激素补充治疗对乳腺的影响。方法 60例绝经后妇女,分为激素组30例,给予小剂量激素(利维爱)补充治疗1~6年;对照组30例。应用超声弹性成像、钼靶X线观察乳腺的形态结构特征,发现乳腺内结节时进行弹性成像评分和应变率比值测量。结果激素组双侧腺体层较对照组厚、导管直径较对照组小,但均无显著性差异(P>0.05);乳腺结构改变超声发生率、钼靶发生率两组比较均无显著性差异(P>0.05),而激素组超声与钼靶发生率比较也无显著性差异(P>0.05)。超声对激素补充治疗(HRT)期间出现新生乳腺实性结节检出率高,弹性成像评分均为1分,弹性应变率比值0.76~1.76。结论超声弹性成像与二维超声联合应用可作为激素治疗期间乳腺监测的首选方法。

【Abstract】 Objective To observe the effects on breast in postmenopausal women under low-dose hormone replacement therapy(HRT) by using ultrasonic elastography(UE) and molybdenum target X-ray photography. Methods 60 postmenopausal women are divided into two groups:HRT group(n = 30) and control group(n = 30). HRT group was given low-dose of Livial for duration from 1 to 6 years.The appearance and structural features of breast were observed by UE and molybdenum target X-ray photography.The elastic score and strain ratio measurement method were used for breast newborn solid nodules.Results The bilateral mammary glandular section thickness of HRT group was thicker than that of the control group,and the bilateral breast duct diameter of HRT group was smaller than that of the control group,but there were no statistical significance(P>0.05).There was no significance (P>0.05) between HRT and control group in the breast structure changes incidence of the subjects by ultrasound,so it was by molybdenum target X-ray photography.There was also no significance(P>0.05) in HRT group in the incidence by ultrasonography and molybdenum target X-ray photography.Ultrasound had high detection rate for appearing newborn solid nodules of breast during HRT,and 1 score and 0.76~1.76 strain ratio for newborn solid nodules by UE.Conclusions Combining UE with two-dimensional ultrasound can be simple and reliable method of mammary gland monitoring during HRT which should be preferred.
