非常规剂量缩宫素对子宫肌瘤血供影响的超声研究

【摘要】 目的研究非常规剂量缩宫素减少子宫肌瘤血供的可行性及其安全性。方法对拟行超声消融治疗的118例子宫肌瘤患者,通过彩色多普勒超声观察肌瘤周边及内部供血动脉在静脉滴注缩宫素(0.12 U./min)前后的变化情况,定量比较使用缩宫素前后肌瘤血供的差异;同时比较主要生命体征的变化。结果静脉滴注缩官素后肌瘤周边及内部供血动脉收缩期峰值流速(PSV)、平均血流速度(Vmean)、直径、血流量减小,而阻力指数(RI)增大,与静脉滴注前比较差异有统计学意义(P<0.05)。静脉滴注前后患者主要生命体征包括心率、收缩压、舒张压、平均动脉压比较,无统计学意义(P>0.05)。结论非常规剂量缩宫素可以安全且有效地减少肌瘤的血供,可能是一种提高超声消融效率的有效方法。

【Abstract】 Objective To study the feasibility of unconventional doses of oxytocin reduce blood supply of uterine fibroids.Methods 118 patients who had uterine leiomyoma,to schedule for HIFU treatment,were included in this study.The most important peripheral and internal artery of fibroid were measured by color doppler ultrasound,including PSV,Vmean,RI,artery diameter,Blood flow.Twenty minutes after intravenous infusion of oxytocinCO.12 U/ min),the identical artery were measured by the same method.Results The hemodynamic parameters of the peripheral and internal artery of fibroid changed obviously after intravenous infusion of oxytocin.The PSV,Vmean,artery diameter, blood flow decreased seriously(P<0.05),while the RI increased significantly(P<0.05).The main vital signs including heart rate,systolic pressure,diastolic pressure,mean arterial pressure,had no statistically significan(P> 0.05).Conclusions The unconventional dose of oxytocinoxytocin is safe it can effectively reduce the blood supply of fibroids,it may be a better way to improve the efficiency of ultrasoud ablation.
