【摘要】 目的观察脐血干细胞移植治疗糖尿病足的临床疗效。方法回顾分析2009年3月至2010年5月本院应用脐血干细胞移植治疗10例糖尿病足患者的临床资料。首先获取脐血200～300ml,从中分离出单个核细胞悬浊液约40ml,行下肢缺血肌肉内局部注射。观察干细胞移植后临床表现及实验室检查指标改善情况,评价疗效。结果移植后大部分患者下肢疼痛、麻木、冷感、间歇性跛行均有不同程度缓解,足部皮温、踝臂指数、经皮氧分压较前明显升高,足部溃疡创面愈合6例,缩小1例。7例动脉造影随访患者均显示有新生侧支血管形成,2肢截小趾,1肢因疼痛、湿性坏疽、合并严重感染而行膝下截肢。结论对于经内科保守治疗无效,不能行血管搭桥术及介入治疗的糖尿病足患者,脐血干细胞移植治疗糖尿病足是一种有效的方法。更多还原

【Abstract】 Objective To investigate the clinical efficacy of cord blood stem cell transplantation on patients with diabetic foot.Methods The clinical data of 10 patients(10 legs) with diabetic foot treated by cord blood stem cell transplantation and admitted in our hospital from March 2009 to May 2010 were retrospectively analyzed.After aspirating 200-300 ml cord blood from the healthy parturients,the mononuclear cells were separated and then injected into the skeletal muscles of the ischemic legs.The improvements of clinical manifestations and laboratory indexes after stem cell transplantation were observed to assess therapeutic effect.Results In most patients,the clinical symptoms and signs including feet pain,numbness,cold feeling and intermittent claudication were alleviated in different extent;Skin temperature in ischemic feet,ankle brachial index and transcutaneous partial pressure of oxygen were all elevated;Ulcer area was healed(6 cases) or was less than before transplantation(1 case);All followed up patients 〔7 patients(7 limbs) 〕who performed angiography showed a noticeable increase in visible collateral vessels.The little toe amputation was performed in 2 cases,and amputation below the knee was performed in 1 case because of pain,moist gangrene and severe infection.Conclusions Cord blood stem cell transplantation could be a simple,safe and effective method for treating patients with diabetic feet,especially for the patients in whom arterial bypass graft and intervention are not feasible.更多还原
