【摘要】 目的:比较不同终止妊娠时间的早发型重度子痫前期患者的妊娠结局,为临床选择最佳终止妊娠时机提供依据。方法:采用回顾性分析的方法对2009～2011年122例早发型重度子痫前期患者的临床资料进行分析,按终止妊娠的孕周分为3组:A组39例(≤32周分娩),B组42例(32～33+6周分娩),C组41例(≥34周分娩),对3组患者的各项临床指标、并发症及妊娠结局进行对比。结果:胎儿生长受限和颅内出血在A、C组间有明显统计学差异(P<0.05),C组胎死宫内和围生儿死亡率均为4.9%,较A、B组发生率低(P<0.05)。DIC、胎盘早剥、心力衰竭、子痫发作、低蛋白血症等方面各组间比较无统计学差异(P>0.05)。C组总体并发症发生率为95.1%,明显高于A组(P<0.05)。结论:早发型重度子痫前期患者孕32周后分娩,围产儿死亡率明显下降,妊娠结局良好。更多还原

【Abstract】 Objective:To compare the pregnancy outcomes of patients with early-onset severe preeclampsia who received pregnancy termination at different opportunities,provide reference for choosing optimal opportunity to terminate pregnancy in clinic.Methods:The clinical data of 122 patients with early-onset severe preeclampsia who were treated in the hospital from 2009 to 2011 were analyzed retrospectively,then the patients were divided into three groups according to the gestational weeks of terminating pregnancy: 39 patients in group A(delivery ≤32 gestational weeks),42 patients in group B(delivery during 32-33+6 weeks),and 41 patients in group C(delivery ≥34 gestational weeks);the clinical indexes,complications,and pregnancy outcomes in the three groups were compared.Results:There was statistically significant difference in the incidences of fetal growth restriction and intracranial hemorrhage between group A and group C(P<0.05).The incidence of intrauterine death and perinatal mortality both were 4.9% in group C,which were statistically significantly lower than those in group A and group B(P<0.05).There was no statistically significant difference in the incidences of DIC,placental abruption,heart failure,onset of eclampsia,and hypoproteinemia among the three groups(P>0.05).The incidence of total complications in group C was 95.1%,which was statistically significantly higher than that in group A(P<0.05).Conclusion:The perinatal mortality of patients with early-onset severe preeclampsia decreases significantly and the pregnancy outcome is good if the patients give birth to their babies after 32 gestational weeks.更多还原
