【摘要】 目的探讨梗阻性左半结肠癌的手术治疗方法,以提高该病的诊治水平。方法总结我院自1992年来手术治疗的梗阻性左半结肠癌的临床资料,按照手术方式不同分为一期手术组和分期手术组,用标准化法比较两组的治愈和复发及5年生存情况,分析手术适应证与手术方式的选择与体会。结果本组58例患者痊愈出院,4例术中因肿瘤发生扩散而行姑息手术,无吻合口瘘和围手术期死亡;随访5年,两组的标化5年生存率分别为60.51%、37.57%,卡方检验差异有统计学意义(χ2=5.83,P=0.016),且一期手术在手术、住院时间及患者所受的痛苦方面均少于分期手术。结论选择合适的病例进行梗阻性左半结肠癌一期切除吻合是可行的,并具有较分期手术更多的时间优势。更多还原

【Abstract】 Objective To evaluate the feasibility of tumor resection for left colon cancer complicated with acute obstruction,in order to improve the technique of treatment.Methods To summary the data of colon cancer patients complicated with acute obstruction from 1992 in our hospital,divided them into two groups by diffident operation way: Grope 1 with stage-Ⅰoperation,and Grope 2 with many stage.Compared the clinical cure rate,recurrence rate and the 5-year survival rate in standardization.To analyze the surgical indications and choice of surgical approach and experience.Results Fifty-eight cases were cured.Four cases were cured by conservative treatment for the tumor had spread on,there were no anastomotic leakage and death in perioperative period.Followed up five years,the two gropes,standardization 5-year survival rate was 60.51%,37.57%.Conclusion It’s safe and feasibility for us to select the proper case in stage I to operate acute obstruction in left colonic carcinoma.Moreover,it can make the patients leave the hospital earlier.更多还原
