【摘要】 目的探讨病灶清除术配合药物用于脊柱结核治疗的临床疗效。方法选择脊柱结核患者56例,经4～6周正规抗结核治疗后行手术治疗。手术方法选择病灶清除椎间植骨融合内固定术,术后行常规抗结核治疗12～18个月。结果①近期疗效:无患者出现脊髓及马尾神经损伤症状,无植骨块移位、脱落及金属内固定断裂,无大血管损伤。②远期疗效,患者治愈率为96.4%。植骨融合成功率为89.3%。43例患者体征消失,占76.8%,12例患者体征减轻,占21.4%,1例症状及体征无明显改善。29例存在脊柱畸形的患者中,基本矫正19例,部分矫正8例,无明显改善2例。③术前平均Cobb角为(17.2±6.8)°,术后平均(10.4±4.3)°,两者比较有统计学意义(P<0.05)。结论病灶清除术配合药物对脊柱结核治疗有良好的疗效,具有很高的应用价值。更多还原

【Abstract】 Objective To study clinical curative effect of drug combining with operation of latibulum clearance for lesions spinal tuberculosis treatment.Methods Selected 56 cases of spinal tuberculosis patients.After 4 to 6 anti-tuberculosis treatment.Surgical method chosen drug combining with operation of latibulum clearance lesions bone graft fixation fusion,postoperative do regular antitubercular treatment 12 to 18 months.Results ①Recent curative effect: no patients experienced the spinal cord and the nerve damage symptom,without cancellous bone piece of shift,fall off and metal fracture fixation,no big vascular injury.②The long-term outcome,patients cure rate was 96.4%.Cancellous bone fusion success rate of 89.3%.43 patients signs disappeared,76.8%,12 patients reduce signs,accounting for 21.4%,and 1 case symptoms and signs have no obvious improvement.29 cases of existence in patients with spinal deformity,basic correction(19 cases),part correct 8 cases,no noticeable improvement in 2 cases.③The average Cobb Angle for preoperative(17.2 ±6.8)°,the average(10.4 ±4.3)°,There was a statistical difference(P < 0.05).Conclusion Drug combining with operation of latibulum clearance lesions in spinal tuberculosis treatment has good curative effect and has the very high application value.更多还原
