超声引导射频消融治疗小肝癌27例的疗效分析

【摘要】 目的总结分析超声引导下应用Cool-tip射频消融系统治疗小肝癌的疗效。方法回顾性分析2009年3月~2011年10月在我院接受射频消融治疗的小肝癌患者27例,其中原发性小肝癌(sHCC)9例,转移性小肝癌(sMHC)18例,共40枚病灶。随访分析术后肿瘤局部消融效果、术后并发症、围术期肝功能变化、生存率等情况。结果术后1、3个月肿瘤局部完全消融率达95.0%(38/40)、90.0%(36/40);患者术后1、2年总生存率分别为96.3%、88.9%;术后无严重并发症发生,围术期肝功能主要指标14 d内基本恢复。结论对于sHCC和sMHC,射频消融可以取得与手术治疗相当的疗效,并具有并发症少、损伤小的优点。

【Abstract】 Objective To summarize and analyze efficacy of ultrasound-guilded treatment for small hepatic carcinoma with Cool-tip radiofrequency ablation(RFA) system.Methods 27 cases of small hepatic carcinoma treated by radiofrequency ablation in our hospital from March 2009 to October 2011 were analyzed retrospectively with 9 cases of small hepatocellu lar carcinoma(sHCC),8 cases of small metastatic hepatic carcinoma(sMHC) and a total of 40 lesions.Postoperative local tumor ablation effect,postoperative complications,perioperative changes in liver function and survival rate were followedup and analyzed.Results Local tumor complete ablation effect rates 1,3 months after treatment were 95.0%(38/40) and 90.0%(36/40);overall survival rates 1,2 years after treatment were 96.3% and 88.9%;no severe postoperative com plication was found,perioperative liver function was basically resumed in 14 days.Conclusion RFA can achieve the equivalent efficacy as the surgical treatment for sHCC and sMHC with fewer complications and damage.
