6酮-前列腺素在糖尿病足患者中的表达及其临床意义
The expression and cinical significance of 6-keto-prostaglandin in patients with diabetic foot
【摘要】 目的检测2型糖尿病(T2DM)并糖尿病足(DF)患者中血浆前列环素(PGI2)的代谢产物6酮-前列腺素(6-Keto-PGF1α)的水平变化,探讨其在DF中的作用和意义。方法 62例T2DM并DF患者分为糖尿病足非坏疽组(A组)35例,糖尿病足坏疽组(B组)27例,35例健康体检者为对照组,记录DF患者的病程,测定DF患者和健康体检者的体质指数(BMI)、血压(BP)、血糖、胰岛素(INS)、糖化血红蛋白(HbA1c)、血脂、胰岛素抵抗指数(HOMA-IR),彩超检测DF患者及健康体检者的踝肱指数(ABI)及内皮依赖性舒张功能(EDD),采用放射免疫法检测血浆6-Keto-PGF1α的含量,并对各变量进行相关分析。结果 DF患者血浆6-Keto-PGF1α水平较对照组显著降低(P<0.01),B组明显低于A组(P<0.01),DF患者血浆6-Keto-PGF1α水平与患者年龄、病程、总胆固醇、低密度胆固醇、血糖、HbA1c、血INS及HOMA-IR水平呈显著性负相关(P<0.05、P<0.01);DF患者ABI、EDD较对照组降低(P<0.01),B组明显低于A组(P<0.05、P<0.01),DF患者的ABI、EDD与血浆6-Keto-PGF1α水平呈正相关(P<0.05、P<0.01)。结论检测血浆6-Keto-PGF1α水平可以早期判断糖尿病患者动脉硬化以及血管内皮细胞舒张功能受损的程度,及早采取干预措施防治糖尿病足的进展。
【Abstract】 Objective To invenstigate the change of 6-keto-prostaglandin F1α(6-Keto-PGF1α) in plasma levels of type 2 diabetes mellitus(T2DM) with diabetic foot and conclude the clinical importance.Methods 62 cases with T2DM were selected and randomly divided into two groups.One was 35 cases with non-gangrenous(group A).The other was 27 cases with gangrenous(group B).The control group was 35 healthy subjects.The duration of DF was recorded.All cases of BMI,BP,blood sugar,insulin,HbA1c,blood fat,and HOMA-IR were collected.All cases of ABI and EDD were tested by color Doppler.The plasma 6-Keto-PGF1α level was measured in all subjects by radio-immunity.The association with plasma 6-Keto-PGF1α level and clinical date were analyzed.Results The results showed that the plasma 6-Keto-PGF1α level was lower(P < 0.01) in DF than those in the control.There were significantly lower(P < 0.01) in group B of the plasma 6-Keto-PGF1α level than those in group A.There were significantly negative association between the plasma 6-Keto-PGF1α levels and age,duration of diabetic mellitus,CHOL,lower density lipoprotein,blood sugar,HbA1c,insulin,HOMA-IR(P < 0.05 or P < 0.01).ABI and EDD were lower(P < 0.01) in DF than those in the control.There were significantly lower(P < 0.05 or P < 0.01) in group B of ABI and EDD than those in group A.ABI and EDD had positive association with the plasma 6-Keto-PGF1α level(P < 0.05 or P < 0.01).Conclusion The detection of plasma 6-Keto-PGF1α can judge the level of atherosclerosis in patients with diabetes and vascular endothelial cells,the degree of diastolic dysfunction early.Measures can be taken in early to prevent the development of DF.
