高危型及低危型人乳头瘤病毒感染在宫颈病变的分布特征

【摘要】 目的研究我院高危型、低危型人乳头状瘤病毒(HPV)在宫颈病变中的感染情况,以探讨宫颈病变与HPV感染的关系。方法采用实时荧光定量PCR法和液基薄层细胞检测技术(LCT)143例女性患者,进行低危型人乳头状瘤病毒(LRHPV6、11)、高危型人乳头状瘤病毒(HRHPV16、18、31、33、45、52、56、58)和宫颈细胞学检测。结果 143例标本中高危型与低危型HPV检出的阳性率分别为30.8%和11.2%。HRHPV感染阳性率从NILM、ASCUS、LSIL、HSIL依次增高。ASCUS组与NILM组的阳性率差异有统计学意义(χ2=5.482,P=0.019);LSIL、HSIL组分别与ASCUS组阳性率比较,其差异也有统计学意义(χ2=14.56,10.725,P=0.01<0.05)。NILM组HRHPV感染病毒载量低于其它三组。结论我院宫颈病变患者中HRHPV感染的阳性率高于LRHPV感染,且随着宫颈病变程度的加重,HRHPV阳性率逐渐增高。HRHPV感染及所带的高病毒载量可导致宫颈病变,但与宫颈病变程度不相关。

【Abstract】 Objective To investigate the situation of human papillomavirus infection in cervical diseases in Putian of Fujian province. and analyze the relationship between different genetypes of HPV or cervical pathologic process. Methods The real time quantitative PCR was used to detect HRHPV and LRHPV and all of the patients received colposcopy with multiple biopsy liquid-based cytology test (LCT). Results The HPV positive rate of high-risk types and low-risk types were 30.8% and 11.2%, respectively (P<0.05). The difference of positivity rates in HRHPV between ASUS and NILM was statistically significant (χ2=5.482, P=0.019). The difference of these four groups in the viral load of HRHPV was statistically significant (χ2=8.712, P=0.033<0.05), and the viral load of HRHPV in NILM group was lower than that of the other three groups. Conclusion Higher viral load could increase the risk of cervical lesions, although there is not relationship between the viral load of HRHPV and the degree of cervical diseases.
