【摘要】 目的总结以心脏占位为主要表现的白塞病患者的临床特点,提高对该病的认识。方法回顾分析7例以心脏占位为主要临床表现的白塞病患者的临床、影像学特点以及治疗和预后。结果 7例患者中男6例、女1例,年龄19～37岁,中位数23岁。临床表现:发热5例,胸痛4例,咯血3例和呼吸困难2例。超声心动图示:占位分布在右心房、右心室4例,左心室1例,双侧心腔内2例。手术切除占位3例,病理显示血栓2例和炎性坏死组织1例。给予华法令、糖皮质激素以及免疫抑制剂治疗后病变消失或缩小。结论白塞病合并心脏血栓多发生于男性青年,更多见于右侧心腔,超声心动图易于发现病变及评价治疗反应,抗凝及免疫抑制治疗有效。更多还原

【Abstract】 Objective To assessment the clinical features in patients with cardiac mass associated with Behcet’s disease. Methods The retrospectively analysis reviewed the clinic-pathologic characteristics,therapy and prognosis of 7 patients with Behcet’ s disease,who were presented mass in cardiac cavity.Results Patients included 6 males and 1 female,ranging from 19 to 37 years old.The main symptoms in these seven patients were fever(5/7),chest pain(4/7),hemoptysis(3/7) and dyspnea(2/7).Upon echocardiography,4 patients showed mass in the right ventricular cavity.In 2 patients,the mass was located in both bilateral cavities,and in one patient in the left ventricular chamber.The histologic appearances of the mass were thrombus in 2 patients,inflammation and necrosis in one patient.It was not recurrent in 2 patients postoperatively and 4 with the mass reduction after immunosuppressive therapy and concomitant anticoagulant.Conclusion Diagnosis of Behcet ’ s disease should be considered if a young man presents with a mass in the cardiac chambers.Echocardiography was helpful to detect and evaluate the change of the mass.It’ s effective by the immunosuppressive therapy and concomitant anticoagulation.更多还原
