脾功能与冠状动脉病变关系的初步探讨
Association of splenic function with severity of coronary artery disease
【摘要】 目的探讨脾功能与冠状动脉病变程度的关系。方法将261例患者按冠状动脉造影结果分为冠心病组(CHD组)和非冠心病组(非CHD组),血涂片镜检豪一周小体计数(HJC),检测目前已知的冠心病的重要危险因素:性别、年龄、吸烟指数、BMI、血压、TC、TG、LDL-C、HDL-C、FPG。比较2组HJC水平差异,对HJC与冠状动脉病变程度的关系进行双因素及多因素分析,对HJC与冠心病传统危险因素进行多因素分析。结果 (1)CHD组血清HJC高于非CHD组(P<0.01)。(2)双变量分析显示冠状动脉病变积分(LNCCS)与HJC呈正相关(r~2=0.07,P<0.01),HJC与TC、LDL-C均呈正相关(r~2=0.04,P<0.01,r~2=0.11,P<0.01),与HDL-C呈负相关(r~2=0.07,P<0.01),而与TG无明显相关性(r~2=0.001,P>0.05)。(3)多元逐步回归分析显示HJC进入回归方程,LNCCS与HJC呈正相关,偏回归系数为0.386,标准化偏回归系数为0.391(P<0.01)。结论 HJC与冠状动脉病变程度呈独立正相关,脾功能减退可能是冠状动脉粥样硬化的独立危险因素。
【Abstract】 Objective To investigate the association of splenic function with severity of coronary artery disease. Methods All 261 patients underwent coronary angiography(CAG) to determine their severity degree of coronary artery di sease.According to the results of CAG,all cases were classified as CHD group and non-CHD group.All the samples underwent blood smear examination,and Howell-Jolly bodies was counted,meanwhile some important traditional risk factors of CHD (gender,age,smoking index,BMI,blood pressure,TC,TG,LDL-C,HDL-C,FPG) were tested.Compare the differences of Howell-Jolly bodies counting(HJC) between the two groups.The relationship of the HJC and CCS was analyzed by two sample test for independent samples,linear correlation and multiple liner stepwise regression.The association of the HJC and other traditional risk factors were analyzed by linear correlation.Results(1)The level of HJC in the CHD group was higher than that in the non-CHD group(P<0.01 ).(2)Linear correlation revealed that the HJC had positive correlation with LNCCS, (r~2=0.07,P<0.01),TC(r~2=0.04,P<0.01)and LDL-C(r~2=0.11,P<0.01),however the level of HJC had negative correlation with HDI.-C(r~2=0.07,P<0.01),and there was no correlation between the level of HJC with TG(r~2= 0.001,P>0.05)(3)Multiple linear stepwise regression analysis showed the level of the HJC enter regression equation, LNCCS had positive correlation with HJC,partial regression coefficient was 0.386,standardized partial regression coefficient was 0.391(P<0.01).Conclusion It demonstrated that the level of the HJC had an independent positive association with severity of coronary artery disease,hyposplenism may be one of the independent risk factors of coronary atherosclerotic.
