【摘要】 目的分析幽门螺杆菌(Hp)感染与胃癌前病变、胃癌的关系。方法对6年中门诊胃镜检查的106 780例患者进行快速尿素酶试验检测Hp,并结合活检病理报告,对Hp感染与年龄、胃癌前病变、胃癌进行回顾性统计分析。结果 Hp感染率由2006年的36.3%降至2011年的24.2%。20-29岁组Hp感染率为34.9%,明显高于40-49岁组的31.4%、50-59岁组的28.5%和>60岁组的24.8%(P<0.05)。6年中,Hp感染患者中肠化生、胃癌发生率均明显下降(P<0.05),高级别上皮内瘤变发生率明显上升(P<0.05)。肠化生、上皮内瘤变、胃癌患者中Hp感染率均显著下降(P<0.05)。结论 6年间胃镜检查患者中Hp感染率逐年下降,Hp感染在青中年中居多,胃癌前病变及胃癌患者的Hp感染率下降。更多还原

【Abstract】 Objective To analyze the relationship of Helicobacter pylori(Hp) infection and gastric precancerous lesions.Methods Hp was detected in 106 780 patients with some symptums of upper gastral tract,who underwent gastroscopy during 6 years period of 2006 to 2011.The relationship of Hp infection and age,intestinal metaplasia(IM),low intraepithelial neoplasia(LIN),high intraepithelial neoplasia(HIN) and gastric cancer(GC) were retrospectively analyzed.ResultsFrom 2006 to 2011,Hp infection rate was decreased from 36.3% to 24.2%.The Hp infection rate was 34.9% in age group of 20-29 years old,which was obviously higher than 31.4% in that of 40-49 years,28.5% in that of 50-59 years and 24.8% in that of >60 years(P<0.05).During 6 years period of 2006 to 2011,the patients with Hp infection,the incidence rates of IM and GC were decreasing(P<0.05),while the incidence of HIN was increasing(P<0.05) and Hp infection rate was obviously decreased in the patients with IM,LIN,HIN and GC(P<0.05).Conclusion During 6 years period of 2006 to 2011,the Hp infection rate is decreased year by year,the patients of 20-29 years old have higher Hp infection rate and the Hp infection rate is decreased in the patients with gastric precancerous lesions and gastric cancers.更多还原
