丙型肝炎病毒感染者的早期肾损伤监测
Monitoring of early renal lesion on patients infected with hepatitis C virus
【摘要】 目的 评价丙型肝炎病毒(HCV)感染者的早期肾损伤。方法 118例HCV感染者按照ALT水平为2个亚组:A组(n=46) ALT <40 U/L,B组(n=72) ALT≥40 U/L;按照HCV感染时间长短分为2个亚组:C组(n=30),明确HCV感染史<5年,D组(n=88)明确HCV感染史≥5年。测定各组HCV感染者尿液中尿微量白蛋白和血清胱抑素C含量,同时与健康体检者(对照组,n=60)进行对照分析,结合肾功能、感染期,评价HCV感染者的早期肾损伤。结果 与对照组相比,A组和B组血清Bun和Cr水平均无明显变化(P>0.05),但血清胱抑素C和尿微量白蛋白水平均明显升高(P<0.05或P<0.01)。B组血清胱抑素C和尿微量白蛋白水平明显高于A组(P<0.05或P<0.01)。与对照组相比,C组和D组血清Bun和Cr水平均无明显变化(P>0.05),但血清胱抑素C和尿微量白蛋白水平均明显升高(P <0.01)。D组血清胱抑素C和尿微量白蛋白明显高于C组(P<0.05或P<0.01)。结论 HCV感染者早期肾损伤严重程度与感染HCV的时间长短及患者的ALT水平有关。检测血清胱抑素C和尿液微量白蛋白应纳入HCV感染所致慢性肾脏疾病(CKD)的常规监测项目。
【Abstract】 Objective To evaluate early renal lesion on patients infected with hepatitis C virus(HCV).Methods118 HCV-infected patients were divided into 2 subgroups based on ALT level:Group A(n=46),ALT <40 U/L and Group B(n=72),ALT≥40 U/L;Then they were divided into another 2 subgroups based on the duration of HCV infection:Group C(n=30),identified history of infection <5 years and Group D(n=88),≥5 years.Urine microalbumin and cystatin C levels of each infection group were examined and compared with healthy subject(control group,n=60),then the evaluation of early renal injuries were made taking consideration of patient’s renal function and duration of HCV infection.Results Compared with control group,the blood urea nitrogen(Bun) and serum creatinine(Cr) levels of Group A and B did not change significantly(P> 0.05),but urine micro-albumin and serum cystatin C levels in both groups significantly increased(P <0.05 or P <0.01),and the levels of group B were significantly higher than Group A(P <0.05 or P <0.01).Compared with control group,the Bun and serum Cr levels of Group C and D did not change significantly(P> 0.05),but urine microalbumin and serum cystatin C levels in both groups significantly increased(P <0.01),and the levels of Group D were significantly higher than group C(P <0.05 or P <0.01).Conclusion The severity of early renal lesion is related to the duration of HCV infection and ALT level of the patients infected with HCV.Examination of urine micro-albumin and serum cystatin C levels should be introduced as routine methods in monitoring HCV-induced chronic kidney diseases(CKD).
