【摘要】 目的观察腹腔镜上消化道溃疡穿孔修补术的临床疗效及其安全性,并总结诊疗经验。方法将2009年4月-2012年2月期间本院收治的98例上消化道溃疡穿孔患者按照手术方式不同分为2组,即腹腔镜组和开腹组,观察2组患者的临床疗效及并发症。结果腹腔镜组手术时间长于开腹组,但差异无统计学意义(P>0.05),而术中出血量显著小于开腹组,差异有统计学意义(P<0.01)。腹腔镜组术后疼痛评分、下床活动时间、排气时间、使用镇痛药例数以及住院天数均显著小于开腹组,差异有统计学意义(P<0.01)。2组患者各出现腹腔残余感染1例。结论腹腔镜上消化道溃疡穿孔修补术疗效确切,安全性好,且术后恢复快,值得推荐。更多还原

【Abstract】 Objective To observe the clinical efficacy and safety of upper gastrointestinal ulcer perforation repair by laparoscope,and to summarize the experience of diagnosis and treatment.Methods Ninety-eight patients with upper gastrointestinal ulcer perforation in our hospital during the period of April 2009 to Feb 2012 were divided into laparoscopic group and laparotomy group according to the surgical approaches.The clinical efficacy and complications of the two groups were observed.Results The operation time in the laparoscopic group was longer than that in the laparotomy group,but the difference didn′t show statistical significance(P>0.05).The amount of blood loss in the laparoscopic group was obviously smaller than that in the laparotomy group,and the difference was statistically significant(P<0.01).The pain scoring,ambulation time,flatulence,number of patients using analgesia and the length of hospital stay in the laparoscopic group were smaller than those in the laparotomy group.The difference showed statistical significance(P<0.01).There was one case of celiac residual infection in each group.Conclusion Upper gastrointestinal ulcer perforation repair by laparoscope has curative effect,high security,rapid postoperative recovery,and it is worth of being recommended in clinical applications.更多还原
