【摘要】 目的:评价多奈哌齐治疗老年性痴呆伴发精神行为异常的临床疗效。方法:将46例老年性痴呆伴发精神行为异常患者分为观察组和对照组各23例,两组均给予降压、降糖、调脂等常规对症治疗药物,观察组在常规药物治疗基础上给予多奈哌齐5 mg,每晚1次,对照组在常规药物治疗基础上给予吡拉西坦每次800 mg,每日3次,两组疗程均为3个月。采用简易智能量表(MMSE)、简明精神病评定量表(BPRS)和日常生活能力量表(ADL)评估治疗1、3个月后患者的认知、精神及生活能力,并比较两组疗效及不良反应。结果:两组患者治疗前MMSE评分比较差异无统计学意义(P>0.05),观察组患者治疗3个月后MMSE评分较治疗前提高(P<0.05),且高于对照组(P<0.05),而对照组治疗后分值无明显改变(P>0.05)。两组患者治疗前BPRS评分比较差异无统计学意义(P>0.05),观察组患者治疗1、3个月后BPRS评分较治疗前降低(P<0.05),且低于对照组(P<0.05),而对照组治疗后分值无明显改变(P>0.05)。两组患者治疗前ADL评分比较差异无统计学意义(P>0.05),观察组患者治疗1、3个月后ADL评分较治疗前降低,且低于对照组,但比较差异无统计学意义(P>0.05),而对照组治疗后分值无明显改变(P>0.05)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论:多奈哌齐可改善老年性痴呆患者认知功能及日常生活自理能力,减轻精神行为异常症状,且安全。更多还原

【Abstract】 Objective: To evaluate the efficacy of Donepezil in the treatment of Alzhelmer’ s disease(AD) with behavioral and psychological symptoms.Methods: Forty-six AD patients with behavioral and psychological symptoms were randomly divided into two groups: observation group(Donepezil 5 mg q.n.) and control group(piracetam 800 mg t.i.d.).The efficacy was assessed by Mini mental state examination(MMSE),Brief psychiatric ratingscale(BPRS),and activity of daily living(ADL) at baseline and 1 and 3 months after the treatment.Adverse events were recorded.Results: There was no significant difference in MMSE between the two groups at baseline(P>0.05).After the treatment of Donepezil for 3 months,significant improvement of cognition of AD patients were observed when comparing with that at baseline or of controls(P<0.05).Meanwhile,significant decline of BPRS was also revealed in observation group(P<0.05).Although no significant difference in BPRS was revealed between the two groups,there were significant alleviations revealed in observation group in 1 and 3 months than those in controls(P<0.05).No significant change of ADL score was revealed no matter between the 2 groups or before and after the treatment.Conclusion: Donepezil effectively improves the cognitive function and ability of daily life,and alleviates behavioral and psychological symptoms in AD patients.更多还原
