【摘要】 目的探讨适宜糖尿病(diabetes mellitus,DM)患者的健康教育管理模式,以提高健康教育效果。方法组建糖尿病俱乐部,对糖尿病俱乐部的100例糖尿病患者实行分阶段达标管理,会员登记制度,提供优质的服务,开展形式多样的健康教育,定期举办知识讲座、联谊会和夏令营活动等,门诊预约随访和电话随访。结果与健康教育前相比,健康教育后糖尿病患者自我管理能力及各项生化指标显著改善(均P<0.01)。结论分阶段达标管理模式为糖尿病患者提供了系统学习糖尿病知识的课堂;个体化教育是强化糖尿病知识,提高自我管理能力的重要方式;预约随访保证了教育的连续性、长期性。更多还原

【Abstract】 Objective To investigate a health education management model suitable for diabetic patients to improve the effects of health education.Methods Setting up the diabetes club,100 cases of diabetes were performed the phased compliance management with member registration system,to provide high quality services,carry out various forms of health education,conduct regular lectures,sodality and summer camp activities,etc.,follow-up outpatient appointments and telephone follow-up.Results After education,the self-management capabilities and the biochemical parameters in diabetic patients were significantly improved as compared with before health education(all P<0.01).Conclusion The phased compliance management model provides a classroom to study diabetic knowledge systematically for diabetic patients;individualized education is an important way to enhance the diabetic knowledge,improve the self-management skills;follow-up appointment assures the continuity and protracted nature of education.更多还原
