【摘要】 目的:探讨肺动脉悬吊的临床特征、诊断与治疗。方法:回顾性分析2008年7月至2011年12月在首都医科大学附属北京安贞医院诊断为肺动脉悬吊患者的临床特征、影像学检查、诊断和治疗。结果:7例肺动脉悬吊患者,男性1例,女性6例,年龄1个月～21岁,<1岁4例,伴有咳喘、肺炎或呼吸困难病史,6例因心脏杂音就诊,经超声心动图、多排螺旋CT明确诊断,2例行纤维支气管镜检查。伴发中位心1例,心内畸形6例,气管、支气管狭窄6例。4例患者手术治疗,3例疗效满意,1例术后早期死亡。结论:儿童中反复出现原因不明的咳喘等呼吸道症状者,应重视肺动脉悬吊的可能,超声心动图联合多排CT可明确诊断,早期行左肺动脉移植术以解除气管狭窄是生存的关键。更多还原

【Abstract】 Objective:To study the clinical features,diagnosis and treatment of pulmonary artery sling.Methods: The clinical manifestations,imaging examinations,diagnostic methods and treatment of children identified as pulmonary artery sling at Beijing Anzhen Hospital from July 2008 to December 2011 were retrospectively reviewed.Results: 7 cases were included,1 male and 9 females,ranging in age from 1 month to 21 years(4 cases less than 1 year of age).All cases had a history of recurrent cough,wheezing,pneumonia or dyspnea,6 cases were diagnosed because of heart murmur.Echocardiography and multidetector CT were done to confirm the diagnoses with mesocardia(1),intracardiac malformations(6),tracheobronchial stenosis(6).2 cases had bronchoscopy examinnations.4 cases underwent surgery and one of them died,the other 3 were good.Conclusion: The children with recurrent respiratory symptoms such as chronic cough and wheeze should be examined for the possible presence of pulmonary artery sling.Echocardiography as a noninvasive technique combined with multi-slice CT can clearly show the anatomy of pulmonary artery sling.Early surgical management of left pulmonary artery translocation to remove the tracheal stenosis is most important to survival.更多还原
