【摘要】 目的探讨右美托嘧啶用于腰硬麻醉下妇科手术病人镇静的临床疗效。方法入选2006年7月～2010年12月在某院妇产科行手术治疗的患者256例,根据术后镇痛药物的不同将患者分为两组,右美托嘧啶组(n=142)和对照组(n=114),比较两组患者患者镇静效果。结果两组患者在术中无1例出现呼吸抑制,比较发现,右美托嘧啶组术中脑电双频普指数、心率分别为65.3±7.2、60.0±9.4,显著低于于术前94.2±5.3、71.2±6.7和对照组术中92.4±6.1、70.0±8.9,差异有统计学意义,P﹤0.05;Ramsay评分术中为4.0±0.0,显著高于术前2.0±0.0和对照组2.0±0.0,差异有统计学意义,P﹤0.05。右美托嘧啶组发生心动过缓11例,占7.7%,低血压9例,占6.3%;对照组心动过缓8例,占7.0%,低血压9例,占7.9%;右美托嘧啶组不良反应发生率为15.0%,与对照组14.9%相比,差异无统计学意义,P﹥0.05。结论右美托嘧啶用于硬膜外麻醉下妇科手术患者镇静安全、有效,可达到满意的镇静效果并且不会发生呼吸抑制。更多还原

【Abstract】 OBJECTIVE To investigate the sedative efficacy of dexmedetomidine in gynecilogical operative patients with spinal anesthesia.METHODS 256 patients were enrolled from July 2006 to December 2010 in obstetrics and gynecology department of our hospital,They were divided into two groups according to the given sedetive drugs,including dexmedetomidine group(n = 142)and control group(n = 112).We compared the sedative efficacy of the two groups.RESULTS None of them showed respiratory depression.The BIS and heart rate were 65.3±7.2 and 60.0±9.4 in dexmedetomidine group,respectively,which was significantly lower than 94.2±5.3 and 71.2±6.7 before operation and 92.4±6.1 and 70.0±8.9 in control group,P﹤0.05.The Ramsay grade in dexmedetomidine group intraoperation was 4.0±0.0 significantly higher than 2.0±0.0 before operation and 2.0±0.0 in control group,P﹤0.05.There were 11 patients showed bradycardia(7.7%)and 9 patients(6.3%)showed low blood pressure in dexmedetomidine group,and 8 patients(7.0%)showed bradycardia and 9 patients(7.9%)showed low blood pressure,P﹥0.05.CONCLUSION Dexmedetomidine is safe and effective in gynecilogical operative patients with spinal anesthesia,which can reaching satisfactory clinical efficacy.更多还原
