【摘要】 目的探讨腹腔镜规则性肝左外叶切除的安全性和有效性。方法腹腔镜下行规则性肝左外叶切除7例,其中肝左外叶巨大囊肿4例、肝左外叶巨大血管瘤1例、肝左外叶肝细胞癌1例、肝左外叶慢性化脓性炎1例。结果 7例患者均成功在腹腔镜下施行了规则性肝左外叶切除术,无中转开腹,手术时间(148.6±79.5)min,术中出血(280.0±218.4)ml,术后胃肠功能恢复(2.6±1.5)d,术后住院(9.1±4.7)d,均无胆漏、出血、腹水、感染、空气栓塞等并发症发生。结论在掌握手术适应证、精心的病例选择及具备丰富的腹腔镜操作经验的基础上,腹腔镜规则性肝左外叶切除是安全、有效的,可充分发挥腹腔镜技术的微创优势。更多还原

【Abstract】 Objective To investigate the safety and effectiveness of regular laparoscopic resection for left hepatic lobe.Methods 7 cases received regular laparoscopic resection for left hepatic lobe,including 4 cases of large liver cyst,1 case of hepatic giant hemangioma,1 case of hepatocellular carcinoma and 1 case of chronic purulent inflammation.And all the 7 foci were located at the left hepatic lobe.Results The operation was successfully performed in the 7 cases.No conversion to laparotomy was needed,nor did any complications occur.The mean surgical time was(148.6±79.5)minutes,the mean blood loss was(280.0±218.4)ml,the mean postoperative recovery time of gastrointestinal function was(2.6±1.5)days,and the mean postoperative hospital day was(9.1±4.7)days.Conclusions Based on the strict surgical indications,careful patient selection and extensive experiences of laparoscopic surgery,regular laparoscopic resection for left hepatic lobe is a safe and effective technique which can give full play to advantages of minimally invasion and cosmetic results.更多还原
