恶性肿瘤患者血小板电泳速度和甲襞微循环中白色微小血栓

【摘要】 目的:观察恶性肿瘤患者血小板数量、电泳速度、聚集功能以及甲襞微循环中白色微小血栓(WMT)和血液流态变化。方法:采集121例恶性肿瘤患者(恶瘤组)手术前抗凝静脉血,分别用血液分析仪检测血小板数量(PLT)、平均体积(MPV)、压积(Pct)和分布宽度(PDW);用细胞电泳技术观测血小板运动速度(PEV);用比浊法检测二磷酸腺苷(ADP)和肾上腺素(ADR)诱导的血小板5min最大聚集率(Ma%);用床旁微循环检测仪检查甲襞微循环中WMT数量和血液流态。并与体检健康人群(对照组)进行比较分析。结果:与对照组比较,恶瘤组PLT、MPV和Pct均显著增加,PDW明显不均,差异具有统计学意义(P<0.05或P<0.01);PEV明显减慢(P<0.05),Ma%显著升高(ADP,P<0.01;ADR,P<0.05)。恶瘤组多数患者出现数量不等WMT,尤以肺癌最多;微循环流态多呈粒流和粒缓流,9例可见间歇性粒摆流。结论:恶性肿瘤血小板计数、功能和微循环检查对观察病情进展(肿瘤转移)和血栓形成有重要作用。

【Abstract】 Objective: To observe platelete counting indexes,electrophoresis velocity,conglomeration function and white microthrombus in nailfold microcirculation of patients with malignant. Method: Platelet counting indexes (PLT,MPV,Pct,PDW) and its electrophoresis velocity(PEV) and max conglomeration ratio(Ma%),as well as white microthrombus(WMT) were examined by corresponding apparatus in 121 patients with malignancies and healthy controls. Results: In malignant patients PLT,MPV,Pct,PDW were increased(P<0.05~0.01),and PEV was slowed down(P<0.05),and Ma% was significantly increased after ADP induction(P<0.01) and ADR induction(P<0.05),and WMT has been seen in majority patients. Conclusion: The changes of the above latoratory data are importment for studing diseases progression tumor metustasis and thrombosis prognostic eveluation.
