【摘要】 目的:探讨中老年人乳腺分泌型癌的临床病理特征、诊断、治疗与预后。方法:对1例64岁女性患者乳腺分泌型癌进行光镜观察、组织化学及免疫组化染色标记,并通过复习相关文献进行分析。结果:乳腺分泌型癌病理形态学特点无年龄、性别差异,均为小管、微囊结构及实性片状区域,以不同形式组合;癌细胞有嗜酸性细胞与透明细胞,体积较大,圆形或多边形,胞浆丰富,核大卵圆形,有小核仁,S-100蛋白强阳性,癌细胞能产生大量分泌物,PAS染色呈阳性反应。结论:乳腺分泌型癌是一种独立类型的乳腺实体肿瘤,根据其形态学特点、组织化学及免疫组化,可以做出明确的病理诊断;儿童及青少年发病,预后好,治疗以局部切除;在男性与中老年女性,预后较差,建议改良或根治为主的手术方式。更多还原

【Abstract】 Objective:To investigate the clinicopathological characteristics,diagnosis,therapy and prognosis of secretory carcinoma of breast in middle-aged women.Methods: The mastectomy specimen was obtained from one case of secretory carcinoma of breast in a 64-year-old female patient and observed by light microscope after histochemical staining and immunohistochemical staining,with reviewing the related literatures for analysis.Results: Secretory carcinoma of breast showed no pathological features for the ages and sexes of patients,but in general demonstrated several histological patterns as solid,microcystic and ductal with combined patterns of the three.The tumor cells contained eosinophils and clear cells that were measured in larger size and shaped roundly or polygonally,with abundant cytoplasm,large oval nucleus and small nucleolus.Still,the cancer cells were strongly positive to S-100 protein and had a large quantity of productions that were positive to PAS staining.Conclusion: Secretory carcinoma of breast is a distinct type of breast solid tumors.Conclusive diagnosis can be morphologically based on its histochemical identification and immunohistochemical test.For patients of juvenile and younger,partial mastectomy is the option and prognosis seems better.Whereas the prognosis is poorer for the middle-aged males and females,thus radical surgical removal is recommended.更多还原
