新疆乌鲁木齐市2009-2011年非结核分枝杆菌药物敏感性试验结果分析

【摘要】 目的:了解和分析新疆乌鲁木齐市3年来非结核分枝杆菌(NTM)的感染趋势、分布及耐药情况和临床特点,为临床治疗NTM病提供科学依据。方法:收集2009年1月至2011年12月3年间新疆乌鲁木齐市患者痰标本分枝杆菌培养阳性的菌株,按照中国防痨协会制定的《结核病诊断细菌学检验规程》进行分枝杆菌菌型鉴定。对鉴定为NTM的菌株采用比例法进行链霉素(SM),异烟肼(INH),利福平(RFP)和乙胺丁醇(EMB)的药物敏感性试验。结果:3年间共收集到3674株分枝杆菌,其中分离出NTM183株,占4.98%。对INH、SM、RFP和EMB的耐药率分别为97.3%、92.9%、73.2%和78.1%。临床主要症状为咳嗽、咳痰、发热、咯血等,其临床症状酷似MTB引致的结核病,一线抗结核药物治疗疗效差甚至无效。结论:NTM肺病的临床特征无特异性,病程长,常规的抗结核药物治疗效果差并呈现高度的耐药性。因此如何解决NTM肺病临床治疗差,是迫在眉睫而又亟待解决的一道重要难题。

【Abstract】 Objective To analysis mycobacterium tuberculosis (NTM) infection trend, distribution and drug resistance and clinical characteristics of nearly 3 years in Xinjiang Urumqi city. Methods To collect from January 2009 to December 2011 in Xinjiang Urumqi city with mycobacterium sputum samples were culture positive strains, Use PNB/TCH identify culture medium strains identified. Identified as NTM strains proportional method for the four a line drug: (SM) streptomycin, isoniazid (INH), rifabutin (RFP) and procedures (EMB) of butyl of drug sensitive test. Results The two years altogether collected from 3674 to plants mycobacteria training content, total separating out the mycobacterium tuberculosis 183 plants, accounted for 4.98%. For INH,SM, RFP and EMB resistant were 97.3%,92.9%,73.2% and 78.1%. Clinical symptoms of cough, main sputum, fever, haemoptysis, etc. The clinical symptoms like mycobacterium tuberculosis tuberculosis, a chemotherapy drug treatment line poor effects or invalid. Conclusion The clinical feature of NTM lung disease nonspecific, the course of a long, conventional antitubercular treatment effect is poor, commonly used drug resistant TB on high. To solve NTM lung disease is more effective treatments.
