磁共振动态增强曲线在孤立性肺结节诊疗中的应用研究
Clinical study of dynamic contrast-enhanced MR curves in diagnosis of solitary pulmonary nodules
【摘要】 目的:评价磁共振动态增强曲线对肺内良、恶性结节鉴别诊断中的应用价值。方法:收集2007年10月至2010年12期间行MR扫描且临床资料完整的孤立性肺结节病例55例,女28例,男27例。所有患者MRI检查包括常规MRI、DCE-MR。依据时间-信号强度曲线的走行情况分为五种类型曲线。结果:A和B型曲线是肺癌的主要曲线类型,占恶性结节的96.55%(28/29),其恶性诊断的灵敏度、特异度、阳性预测值、阴性预测值、正确指数分别是73.68%、94.12%、96.55%、61.54%、0.678。D和R型曲线是良性结节的主要曲线类型,R型曲线以结核球和错构瘤为主,结核球以R1型曲线为主,而错构瘤以R2型曲线为主。结论:肺结节的DCE-MR强化曲线可分为5种类型,并对肺结节的良、恶性鉴别诊断具有一定的意义。
【Abstract】 Objective The aim of our study was to investigate the application value on dynamic contrast-enhanced MR (DCE-MR) imaging and its curves for differential diagnosis between benign and malignant pulmonary lesions. Methods From 2007 October to 2010 December, fifty-five consecutive patients were prospectively enrolled in the study, twenty-eight women and twenty-seven men. The imaging sequences were as follows: routine MR imaging and DCE-MR imaging. Five types of time signal intensity curve (TIC) were classified according to enhancement conditions. Results A and B type curves were main curve type for lung cancer. D and R type curves were main curve type for benign nodules. R type curve mainly included lung tuberculoma and harmatoma. Moreover, lung tuberculoma mainly displayed R1 curve and harmatoma mainly displayed R2 curve. Conclusion Time signal intensity curves of lung nodules were classified to five types in our study and were valuable for differential diagnosis of benign and malignant pulmonary lesions.
