【摘要】 目的:探讨"医院-社区一体化康复"服务模式对慢性精神分裂症患者疗效。方法:610例慢性精神分裂症按照其是否参与"医院-社区一体化康复"分为康复组304例和对照组306例,对照组进行药物治疗,康复组除药物治疗外,同时参与集体家庭干预、社交技能训练和职业康复训练。两组患者每半年进行一次病情、精神症状、自知力、疗效和社会功能评估,两年后比较两组患者的疗效。结果:康复组病情明显好于对照组(χ2=12.307,P<0.05);康复组的精神症状明显少于对照组;自知力比较,康复组自知力明显好于对照组(χ2=12.992,P<0.01);康复组治疗效果明显好于对照组(χ2=12.975,P<0.05)。康复组工作能力明显好于对照组,完全或部分胜任工作或承担全部家务的人比对照组多,而丧失工作能力或承担家务的人明显少于对照组(χ2=14.253,P<0.05)。康复组残疾功能评定量表Ⅱ总分及各因子分均明显低于对照组(t=10.495,P<0.01)。结论:"医院-社区一体化康复"治疗能明显改善慢性精神分裂症患者的病情、控制精神症状、帮助恢复自知力和改善其社会功能,有助于他们早日回归社会。更多还原

【Abstract】 Objective To explore the efficacies of ’rehabilitation of hospital-community integration’ treatment model for patients with chronic schizophrenia in Shenzhen.Methods 610 patients with chronic schizophrenia were divided into rehabilitation group(n = 304) and control group(n = 306) according to the participation of ’rehabilitation of hospital-community integration’.Patients in the control group was treated with drugs,and patients in the rehabilitation group received drugs and trainings of family intervention,social skills,and vocational rehabilitation.Psychiatric symptoms,insights,efficacies,and social functions were assessed at every six months,and the outcomes of the two groups were compared 2 years later.Results The disease condition,insights,and the efficacies in rehabilitation group were significantly better than those in control group(χ2 = 12.307,χ2 = 12.992,and χ2 = 12.975,all P < 0.05).The psychiatric symptoms and the total and factors scores of disability function Ⅱ in rehabilitation group were significantly lower than those in control group.The number of people who could wholly or inpart do the work or household in rehabilitation group was significantly higher than that in control group,while the number of people who lost the ability to work or household duties was significantly less(χ2 = 14.253,P < 0.05).Conclusions ’Rehabilitation of hospital-community integration’ treatment can improve the condition of patients with chronic schizophrenia,the control of psychiatric symptoms,and the insight to help restore and improve their social function,and can help them return to society as soon as possible.更多还原
