64层螺旋CT泪囊造影在评价泪系病变中的临床应用

【摘要】 目的分析64层螺旋CT泪囊造影表现,评价其临床应用价值。方法回顾分析2007年11月至2010年11月间进行CT泪囊造影检查的33例泪溢患者之影像资料。结果本组33例45眼泪囊造影均获得了成功,根据泪道阻塞的有无及阻塞部位的不同,CT表现可分为4型:Ⅰ型泪道无阻塞(9眼),泪小管、泪囊、鼻泪管及同侧鼻腔显影;Ⅱ型鼻泪管下段阻塞(11眼),泪小管、泪囊及部分鼻泪管显影;Ⅲ型鼻泪管上段阻塞(19眼),泪小管、泪囊显影;Ⅳ型泪囊及以上阻塞(6眼),仅泪小管显影。结论 CT泪囊造影可直观显示泪道有无阻塞及阻塞部位,有助于治疗方案的选择及预后判断。

【Abstract】 Objective To analyze the imaging findings of lacrimal system lesions on 64-slice sprial CT dacryocystography and to evaluate their clinical significance.Methods Imaging data of 33 patients with epiphora,who underwent 64-slice spiral CT dacryocystography from November 2007 to November 2010,were retrospectively analyzed.Results 45 eyes of 33 patients were successfully visualized on CT dacryocystography.According to whether or not lacrimal passage obstruction and obstructive positions on CT findings,all patients could be categorized into the following four types: typeⅠ,lacrimal passage without obstruction(9 eyes),appeared lacrimal canal,dacryocyst,nasolacrimal duct and ipsilateral nasal cavity visualization;typeⅡ,obstruction of lower segment nasolacrimal duct(11 eyes),appeared lacrimal canal,dacryocyst and part nasolacrimal duct visualization;typeⅢ,obstruction of upper segment nasolacrimal duct(19 eyes),appeared lacrimal canal and dacryocyst visualization;TypeⅣ,obstruction of dacryocyst and above(6 eyes),appeared only lacrimal canal visualization.Conclusion 64-slice spiral CT dacryocystography could directly and stereoscopically show whether or not lacrimal passage obstruction and obstructive position,so that it is very beneficial to select the thearpeutic protocol and to assess the prognosis.
