人小细胞肺癌原位瘤与皮下瘤模型影像及病理对照研究
Comparison study of imaging and pathological features between the orthotopic transplantation tumor and the subcutaneous tumor model of human small cell lung cancer
【摘要】 目的 比较人小细胞肺癌原位瘤与皮下瘤模型影像学及组织病理学表现差异。方法 40只裸鼠随机分为原位瘤组和皮下瘤组,将人小细胞肺癌(NCI-H446)细胞悬液分别种植于各组裸鼠左肺内与左腋皮下构建肺癌模型。通过CT、MRI及病理比较2种模型表现异同。结果 原位瘤组CT及MRI发现16只成瘤,CT呈结节样高密度,T1WI呈略低信号,T2WI呈高信号;皮下瘤组CT及MRI均发现病灶,CT呈近似等密度,T1WI信号与胸壁类似,T2WI呈不均匀高信号,周围见低信号包膜。原位瘤组病理显示16只成瘤,肿瘤呈浸润性或伴膨胀性生长,细胞形态及分化类似人小细胞肺癌,瘤内见新生血管,9只模型发现肺内及纵隔转移;另1只模型肺内病灶呈粟粒样,而CT及MRI均未发现。皮下瘤组病理显示20只全部成瘤,肿瘤呈膨胀性生长,周围有纤维包膜,细胞有向腺癌方向分化趋势,瘤灶内发现坏死,未见新生血管。结论 原位瘤及皮下瘤在组织病理、生物学特点及影像学表现具有明显差异;而原位瘤模型更多地保留了人体内小细胞肺癌原有特征。
【Abstract】 Objective To compare the imaging and pathological features between the orthotopic and subcutaneous transplantation model of the human small cell lung cancer.Methods Forty mice were divided into orthotopic and subcutaneous groups randomly.The cultured human small cell lung cancer cells(NCI-H446) were planted into the left lung or under the left armpit skin of the mice to establish tumor model.The differences of CT,MRI and pathology between the two groups were compared.Results Lesions were detected in 16 cases in orthotopic group and 20 cases in subcutaneous group by CT and MRI respectively.In orthotopic group,lesions were hyperdensity on plain CT,slightly hypointense on T1WI,and slightly hyperintense on T2WI.In subcutaneous group,lesions were slightly hypo-or isodensity on plain CT,slightly isointense on T1WI,hyperintense on T2WI,while hypointense capsules were detected on T2 WI.In orthotopic group,pathological result revealed the tumor grew in invasive or/and expansive way in 16mice.Cancer cells had the characteristic of small cell lung cancer,and new capillaries were detected.Lung and mediastinal metastases were found in 9 cases,while lesions were missed by CT and MRI in one case.In subcutaneous group,pathological findings included the characteristic of cancer cells alike adenocarcinoma in 20 mice,and the tumors grew in expansive way with capsule of fiber.Necrosis were detected,while no new capillaries were detected.Conclusion The orthotopic and subcutaneous transplantation tumor model own their special characteristics of pathology,biology and image.Moreover,the orthotopic model kept more characteristics of small cell lung cancer of human.
