磁共振双反转角T1 map在膝关节软骨退变中的应用价值

【摘要】 目的 探讨磁共振T1 map在膝关节软骨退变中的临床应用价值。方法 41例受检者共41例膝关节分为正常组(n=11)、轻度骨性关节炎(OA)组(n=20)和重度(OA组(n=10),经肘静脉注射对比剂Gd-DTPA2-90~120 min后行3.0T磁共振扫描,采用双反转角T1map技术及后处理软件得到T1图,分别测量股骨内、外侧髁,胫骨内、外侧髁等4处软骨T1值并对各组间差异进行统计学分析。正常组、轻度OA患者、重度OA患者组间T1值的比较采用q检验。结果 正常组、轻度OA组及重度OA组平均关节软骨T1值分别为518.9~540.2ms、465.0~482.7ms、65.9~370.2 ms。正常组与OA组T1值之间均存在显著性差异(P<0.001),轻度OA组与重度OA组T1值之间也存在显著性差异(P<0.001)。结论MR T1 map可用于评价关节软骨生化成分的改变,并在早期关节软骨退变的诊断中具有较好的临床应用价值。

【Abstract】 Objective To evaluate the clinical application of MRI T1 map in knee osteoarthritis(OA).Methods 41 patients were divided into 3 groups:normal group(n=11),mild OA group(n=20) and severe OA group(n=10).They were all underwent3.0T MRI examination.90-120 min after Gd-DTPA2- intravenous injected,T1W images were obtained by using dual-angle T1map technique and postprocessing software.The T1 values of cartilage were measured at condylus medialis femoris,condylus lateralis femoris,condylus medialis tibial plateau and condylus lateralis tibial plateau.Statistical significance was inferred at P<0.05.Results The T1 values of arthrodial cartilage in normal group,mild OA group and severity OA group were 518.9-540.2 ms,465.0-482.7 ms and 365.9-370.2 ms,respectively.There were significant differences among the three groups(P<0.001).Conclusion MR T1 map can be used in evaluating the articular cartilage changes and the biological composition.It plays an important role in the early diagnosis of cartilage degeneration.
