640层CT冠状动脉成像对比剂用量对冠状静脉窦的影响
Effect of contrast agent volume on coronary sinus in computed tomography coronary angiography using a 640-slice scanner
【摘要】 目的 分析CT冠状动脉造影不同对比剂用量对冠状动脉及冠状静脉窦强化程度的影响。方法 70例行640层CT冠状动脉成像的低心率(≤65次/min)连续病例根据对比剂用量不同分为2组:A组35例,根据体重按0.7 mL/kg注射造影剂,固定注射时间10 s;B组35例,根据体重按0.8 mL/kg注射对比剂,固定注射时间12 s。所有病例注射对比剂后,注射30 mL生理盐水进行冲刷。测量并记录升主动脉、降主动脉、左冠状动脉主干开口、右冠状动脉开口、冠状静脉窦的CT值。结果 A组和B组的年龄、身高、体重、心率等一般影响因素无统计学差异。A组的对比剂用量显著低于B组(P<0.05)。2组平均对比剂注射速率(4.7mL/s vs 4.5 mL/s)和CTA增强时间(23.4 s vs 23.7 s)无统计学意义(P>0.05)。2组动脉强化程度(升主动脉、降主动脉、左冠状动脉主干开口和右冠状动脉开口处的CT值)无统计学意义(P>0.05)。2组静脉窦强化程度(93.7 HU vs 112.7 HU)有统计学差异(P<0.05)。结论 根据体重调节640层CT冠状动脉成像的对比剂用量是可行的,0.7 mL/kg方案具有较高的冠状动脉强化和较低的静脉窦显影,对比剂使用效率更高。
【Abstract】 Objective To investigate the effect of contrast agent volume on coronary artery and coronary sinus in computed tomography coronary angiography(CTCA) using a 640-slice scanner.Methods According to the volume of contrast agent,seventy patients with low heart rate(≤65 times/min.) who underwent CTCA in a single heart beat with electrocardiogram-gating were divided into two groups.Patients in group A(n=35) were received 0.7 mL/kg of contrast agent injected at a fixed duration of 10 seconds,and those in group B(n=35) were received 0.8 mL/kg of contrast agent injected at a fixed duration of 12 seconds,followed by 30mL of saline injected in all patients.Contrast densities(CT attenuations) were assessed at the ascending aorta,descending aorta,the ostia of the left main coronary artery(LMA) and the right coronary artery(RCA),and coronary sinus(CS) were measured.Results There were no significant differences in age,height,weight,and mean heart rate between group A and group B.The mean total dose of contrast material in group A was significantly lower than that in group B(P<0.05).There were no significant differences in the mean flow rate(4.7 mL/s in group A,4.5 mL/s in group B) and the mean contrast-enhancement(23.4 s vs 23.7 s).There were no significant differences in the mean enhancement values of the ascending aorta,descending aorta,the ostia of LMA and RCA(P>0.05).There were significant differences in the mean enhanced values of CS between two groups(P<0.05).Conclusion Less contrast agent can be used with the protocol of 0.7 mL/kg contrast agent injected at a fixed duration of 10 seconds in CTCA using a640-slice scanner,with a reliable contrast-enhancement in the coronary artery and less visualization in coronary sinus.
