兔膝关节骨软骨骨折转归的病理学与MRI表现对照研究
Contrast study of osteochondral fracture development of knee in rabbit:pathology and MRI findings
【摘要】 目的 研究骨软骨骨折转归过程的病理学变化与MRI表现关系。方法 随机选用20只月龄及体重相近的健康新西兰大白兔,雌雄不限;建立兔膝股骨髁原位较稳定的骨软骨骨折(OCF)模型,所有模型均使用屈膝30°位长腿石膏外固定。术后1、2周分别从试验组中随机抽取10只实验兔行MRI扫描、病理组织学检查。MRI扫描序列包括:TSE-T1WI、TSE-T2 WI、T2WI/SPAIR、3D/WATSc/FFE/T1 WI。结果 MRI表现:术后1周,骨折线清晰9只(9/10)、模糊1只(1/10);术后2周,骨折线均模糊(10/10)。病理检查:骨折线清晰的成纤维细胞数多,骨折线模糊的成纤维细胞数少,且骨折区有大量新生骨小梁。骨折区成纤维细胞数术后1周与术后2周比较P<0.05。MRI表现与病理检查骨片覆盖软骨损伤等级结果比较P>0.05。结论 MRI对于膝关节骨软骨骨折转归的观察与病理对照有较好一致性。
【Abstract】 Objective To study the relativity of MRI features and pathology of osteochondral fracture development of rabbit ’s knee.Methods Twenty young healthy New Zealand rabbits with similar age and body weight were chosen randomly and the sexuality was neglected.The osteochondral fracture models(OCF) in situ were made.All the models were given external fixation of the gypsum in situ of 30°.Ten experimental rabbits were chosen randomly and underwent MRI examination and pathologic examinations after one week and two weeks,respectively.MR imaging sequences included:turbo spin echo T1 weighted image(TSE-T1WI),turbo spin echo T2 weighted image(TSE-T2 WI),Spin-spin relaxation time weighted image/spectral attenuated inversion(T WI/SPAIR),three dimensional/water selective/fast field echo/Spin-lattice relaxation time weighted image(3D/WATSc/FFE/T1 WI).Results A fter one week of operation,MR imaging shwoed that the fracture lines were clear in 9(9/10) and undefinite in 1(1/10).After two weeks of operation,all fracture lines were undefinite(10/10).Histopathological observation showed that there were more collagenoblast cells in the fracture zones with clear margins compared to the bone fracture zone with undefinite margins,and presented bone trabeculaes.There was statistical significance in the number of collagenoblast cells between one week and two week(P<0.05) after operation.There was no significant difference between the lesion number of obducent cartilage in bone flaps observed by MRI and by histopathology(P<0.05).Conclusion There is better concordance in detecting the progression of osteochondral fracture of the knee in rabbits between MRI and pathology.
