【摘要】 目的 探讨高分辨率CT(HRCT)在多发性肌炎皮肌炎肺损害诊断中的应用。方法 回顾性分析23例经临床证实的多发性肌炎/皮肌炎肺损害HRCT检查资料,其中男7例,女16例,对其表现特征进行分析,并对照分析治疗前后HRCT表现。结果23例多发性肌炎/皮肌炎肺损害病人中10例(10/23)可见两肺内磨玻璃密度影,16例(16/23)显示小叶间隔、小叶内隔线增厚,5例(5/23)出现支气管血管束增厚,17例(17/23)出现胸膜下线影,3例(3/23)有实变,9例(9/23出现网格状、蜂窝状影,11例(11/23)胸膜肥厚,3例(3/23)胸腔积液,5例(5/23)出现纵隔淋巴结增大。病变多分布于胸膜下区域和肺底部。治疗前后HRCT变化与临床表现一致。结论 HRCT可以显示多发性肌炎皮肌炎肺损害的细微病变,对多发性肌炎/皮肌炎肺损害的诊断具有重要价值,可以客观地评价临床疗效。更多还原

【Abstract】 Objective To explore the clinical application of high resolution CT(HRCT) for pulmonary involvement in polymyositis(PM) and dermatomyositis(DM).Methods HRCT data of 23 cases(7 men and 16 women) with PM and DM proved by clinic were retrospectively reviewed.HRCT features before and after the treatment were comparatively analyzed.Results Of all 23 cases,HRCT findings included:ground-glass opacities in 10(10/23) cases,interlobular septal thickening in 16(16/23),irregular peribronchovascular thickening in 5(5/23),parallel subpleural lines in 17(17/23),consolidation in 3(3/23),honeycombing in 9(9/23),pleural thickening in 11(11/23),pleural effusion in 3(3/23) and mediastinal lymphadenovarix in 5(5/23).These findings were more remarkable in lower lung peripheral zones.The clinical manifestations are consistent of HRCT changes before and after treatment.Conclusion HRCT can show the details of pulmonary involvement in PM and DM,which is of very important values in evaluating the clinical efficacy for this disease.更多还原
