利用PET-CT勾画食管癌靶区对放射性肺损伤的影响
Influence of using PET-CT delineated esophageal carcinoma target for radiation-induced lung injury
【摘要】 目的:探讨CT扫描与PET-CT扫描在勾画食管癌靶区放疗后放射性肺损伤的差异。方法:利用PET-CT影像与同机的CT影像分别勾画14例胸中段食管癌患者的靶区体积,并利用Pinnacle3治疗计划系统分别计算出肺V20、V30和平均肺剂量;同期另15例胸中段食管癌患者由CT影像勾画靶区体积;并计算出肺V20、V30和平均肺剂量;29例患者放疗结束后3月、6月行CT扫描,计算放射性肺损伤的体积。结果:14例利用PET-CT勾画均小于利用同机CT勾画的靶区体积、肺V20、V30和平均肺剂量较小,放射性肺损伤发生率21%;另15例利用CT勾画的靶区体积、肺V20、V30和平均肺剂量较大,放射性肺损伤发生率40%,且放射性肺损伤体积大于利用PET-CT扫描在勾画食管癌靶区的患者。结论:利用PET-CT勾画食管癌放疗靶区优于CT,可减少放射性肺损伤。
【Abstract】 Objective:To investigate the variance in radiation-induced lung injury between the tumor target volume delineated by CT scan and that delineated by positron emission tomograpgy and computed tomograpgy(PET-CT)scan in esophageal carcinoma.Methods: The target volume of 14 patients with middle thoracic esophageal cancinoma were delineated by PET-CT imaging and delineated by CT imaging in the same machine,the dose-volumes of lung V20,V30and mean lung dose(MLD) were calculated by Pinnacle3 treatment planning system(TPS);the target volume of 15 patients with middle thoracic esophageal cancinoma were delineated by CT imaging in the same period,and calculate lung V20,V30 and MLD by TPS;the chest of all 29 patients were CT scaned at 3 months,6 months after the end of radiotherapy,and the volume of radiation-induced lung injury were calculated by TPS.Results: The target volume and lung V20,V30 and MLD of 14 cases delineated by PET-CT were less than by CT scan in the same machine,the occurrence rates of radiation-induced lung injury was 21%;The target volume and lung V20,V30 and MLD of other 15 cases delineated by CT scan were greater than by PET-CT scan,the occurrence rates of radiation-induced lung injury was 40%,the volume of radiation-induced lung injury volume in patients with esophageal cancer target delineated by CT scan was greater than those by PET-CT scan.Conclusion: PET-CT is superior to CT in delineating the target volume and reduce occurrence rates of radiation-induced lung injury in the patients with esophageal carcinoma.
